MOVEMENT-BASED MINDFULNESS INTERVENTIONS (MBI) ARE COMPLEX, MULTI-COMPONENT INTERVENTIONS FOR WHICH THE DESIGN PROCESS IS RARELY REPORTED. FOR PEOPLE WITH STROKE, EMERGING EVIDENCE SUGGESTS BENEFITS, BUT MAINSTREAM PROGRAMS ARE GENERALLY UNSUITABLE. WE AIMED TO DESCRIBE THE PROCESSES INVOLVED AND TO CONDUCT A FORMATIVE EVALUATION OF THE DEVELOPMENT OF A NOVEL YOGA-BASED MBI DESIGNED FOR SURVIVORS OF STROKE. WE USED THE MEDICAL RESEARCH COUNCIL COMPLEX INTERVENTIONS FRAMEWORK AND PRINCIPLES OF CO-DESIGN. WE PURPOSEFULLY APPROACHED HEALTH PROFESSIONALS AND CONSUMERS TO ESTABLISH AN ADVISORY COMMITTEE FOR DEVELOPING THE INTERVENTION. MEMBERS COLLABORATED AND ITERATIVELY REVIEWED THE DESIGN AND CONTENT OF THE PROGRAM, FORMATTED INTO A TRAINING MANUAL. FOUR EXTERNAL YOGA TEACHERS INDEPENDENTLY REVIEWED THE PROGRAM. FORMATIVE EVALUATION INCLUDED REVIEW OF MULTIPLE DATA SOURCES AND DOCUMENTATION (E.G., FORMAL MEETING MINUTES, FOCUS GROUP DISCUSSIONS, RESEARCHER OBSERVATIONS). THE DATA WERE SYNTHESIZED USING INDUCTIVE THEMATIC ANALYSIS. THREE BROAD THEMES EMERGED: (A) MBI CONTENT AND TERMINOLOGY; (B) MANUAL DESIGN AND READABILITY; AND (C) BARRIERS AND ENABLERS TO DELIVER THE INTERVENTION. VARIOUS PERSPECTIVES AND FEEDBACK ON ESSENTIAL COMPONENTS GUIDED FINALIZING THE PROGRAM. THE DESIGN PHASE OF A NOVEL YOGA-BASED MBI WAS STRENGTHENED BY INTERDISCIPLINARY, CONSUMER CONTRIBUTIONS AND PEER REVIEW. THIS STUDY FOCUSED ON THE PERSPECTIVES OF SCHOOL PERSONNEL AFFILIATED WITH THE ENCINITAS UNION SCHOOL DISTRICT IN CALIFORNIA FOLLOWING A LAWSUIT ARGUING THAT THEIR YOGA-BASED PROGRAM INCLUDED RELIGION AND THEREFORE WAS UNSUITABLE FOR IMPLEMENTATION IN PUBLIC SCHOOLS AND WAS UNCONSTITUTIONAL. PARTICIPANTS (N = 32) WERE INTERVIEWED USING A SEMISTRUCTURED INTERVIEW, AND DATA WERE ANALYZED ACCORDING TO INTERPRETATIVE PHENOMENOLOGICAL ANALYSIS. FIVE SUPER-ORDINATE THEMES (INCLUDING SUB-THEMES) WERE IDENTIFIED IN AN ITERATIVE PROCESS, INCLUDING: PARTICIPANTS' PERSPECTIVES ON THE ROOTS OF YOGA AND THE TYPE OF YOGA TAUGHT IN THEIR DISTRICT; THE PROCESS OF INTRODUCING A YOGA-IN-THE-SCHOOLS PROGRAM IN LIGHT OF THIS CONTENTION (INCLUDING CHALLENGES AND OBSTACLES, AND HOW THESE WERE MET); PERSPECTIVES ON THE LAWSUIT AND HOW THE PROCESS UNFOLDED; EFFECTS OF THE LAWSUIT ON SCHOOL CLIMATE AND BEYOND; AND PERSPECTIVES ON YOGA AS, AND AS NOT, RELIGIOUS. THE AIM OF THIS STUDY WAS TO EXPLORE THE FEASIBILITY OF RESEARCHING COMMUNITY BASED YOGA CLASSES IN TYPE 2 DIABETES WITH A VIEW TO INFORMING THE DESIGN OF A DEFINITIVE, MULTI-CENTRE TRIAL METHODS: THE STUDY DESIGN WAS AN EXPLORATORY RANDOMISED CONTROLLED TRIAL WITH IN-DEPTH PROCESS EVALUATION. THE SETTING WAS TWO MULTI-ETHNIC BOROUGHS IN LONDON, UK; ONE WITH AVERAGE AND ONE WITH LOW MEAN SOCIO-ECONOMIC DEPRIVATION SCORE. CLASSES WERE HELD AT A SPORTS CENTRE OR GP SURGERY. PARTICIPANTS WERE 59 PEOPLE WITH TYPE 2 DIABETES NOT TAKING INSULIN, RECRUITED FROM GENERAL PRACTICE LISTS OR OPPORTUNISTICALLY BY GENERAL PRACTICE STAFF. THE INTERVENTION GROUP WERE OFFERED 12 WEEKS OF A TWICE-WEEKLY 90-MINUTE YOGA CLASS; THE CONTROL GROUP WAS A WAITING LIST FOR THE YOGA CLASSES. BOTH GROUPS RECEIVED ADVICE AND LEAFLETS ON HEALTHY LIFESTYLE AND WERE ENCOURAGED TO EXERCISE. PRIMARY OUTCOME MEASURE WAS HBA1C. SECONDARY OUTCOME MEASURES INCLUDED ATTENDANCE, WEIGHT, WAIST CIRCUMFERENCE, LIPID LEVELS, BLOOD PRESSURE, UKPDS CARDIOVASCULAR RISK SCORE, DIABETES-RELATED QUALITY OF LIFE (ADDQOL), AND SELF-EFFICACY. PROCESS MEASURES WERE ATTENDANCE AT YOGA SESSIONS, SELF-REPORTED FREQUENCY OF PRACTICE BETWEEN TAUGHT SESSIONS, AND QUALITATIVE DATA (INTERVIEWS WITH PATIENTS AND THERAPISTS, ETHNOGRAPHIC OBSERVATION OF THE YOGA CLASSES, AND ANALYSIS OF DOCUMENTS INCLUDING MINUTES OF MEETINGS, CORRESPONDENCE, AND EXERCISE PLANS). RESULTS: DESPITE BROAD INCLUSION CRITERIA, AROUND TWO-THIRDS OF THE PATIENTS ON GP DIABETIC REGISTERS PROVED INELIGIBLE, AND 90% OF THE REMAINDER DECLINED TO PARTICIPATE. MEAN AGE OF PARTICIPANTS WAS 60 +/- 10 YEARS. ATTENDANCE AT YOGA CLASSES WAS AROUND 50%. NOBODY DID THE EXERCISES REGULARLY AT HOME. YOGA TEACHERS FELT THAT MOST PARTICIPANTS WERE UNSUITABLE FOR 'STANDARD' YOGA EXERCISES BECAUSE OF LIMITED FLEXIBILITY, LACK OF BASIC FITNESS, CO-MORBIDITY, AND LACK OF CONFIDENCE. THERE WAS A SMALL FALL IN HBA1C IN THE YOGA GROUP WHICH WAS NOT STATISTICALLY SIGNIFICANT AND WHICH WAS NOT SUSTAINED SIX MONTHS LATER, AND NO SIGNIFICANT CHANGE IN OTHER OUTCOME MEASURES. CONCLUSION: THE BENEFITS OF YOGA IN TYPE 2 DIABETES SUGGESTED IN SOME PREVIOUS STUDIES WERE NOT CONFIRMED. NEVERTHELESS, AMIDST ITS RISE ARE CONCERNS ABOUT YOGA'S DEPARTURE FROM ITS SPIRITUAL FOUNDATIONS AND INCREASING ASSIMILATION INTO THE APPEARANCE- AND COMMERCIAL-DRIVEN EXERCISE AND FITNESS CULTURE. CONSEQUENTLY, AN EXCLUSIONARY IDENTITY HAS BEEN PERPETUATED IN SHAPING NORMS SURROUNDING WHO CAN AND DOES PRACTICE YOGA, WHICH CONTRADICTS EARLIER EGALITARIAN VISIONS OF A YOGA FOR ALL. THEREFORE, WE ADOPT A SOCIAL JUSTICE LENS IN OFFERING A FOCUSED ANALYSIS OF THE INTERSECTION OF YOGA, EMBODIMENT, AND INCLUSION FOR SELECT MARGINALIZED SOCIAL IDENTITIES TYPICALLY UNDERREPRESENTED AMONG YOGA PRACTITIONERS AND IN YOGA SCHOLARSHIP. DATA ARE SYNTHESIZED FROM BOTH QUALITATIVE AND QUANTITATIVE SOURCES AND INTEGRATE AN UNDERSTANDING OF HOW CONFINED MEDIA REPRESENTATIONS OF "THE YOGA BODY" AND OTHER PRACTICAL CONSTRAINTS MAY UNDERMINE THE PERCEIVED ACCESS TO THE PRACTICE FOR MEMBERS OF DIVERSE GROUPS. WE CONCLUDE WITH INVITING FUTURE CONSIDERATIONS TOWARDS FOSTERING MORE INTERDISCIPLINARY COMMUNITY-BASED RESEARCH PARTNERSHIPS AMONG THE VARIETY OF STAKEHOLDERS INVESTED IN ADVANCING THE ACCESSIBILITY AND INCLUSION OF YOGA AND POSITIVE EMBODIMENT FOR ALL BODIES. 2020 12 2763 24 YOGA PROGRAM FOR TYPE 2 DIABETES PREVENTION (YOGA-DP) AMONG HIGH-RISK PEOPLE: QUALITATIVE STUDY TO EXPLORE REASONS FOR NON-PARTICIPATION IN A FEASIBILITY RANDOMIZED CONTROLLED TRIAL IN INDIA. BACKGROUND: YOGA-BASED INTERVENTIONS CAN BE EFFECTIVE IN PREVENTING TYPE 2 DIABETES MELLITUS (T2DM). WE DEVELOPED A YOGA PROGRAM FOR T2DM PREVENTION (YOGA-DP) AMONG HIGH-RISK PEOPLE AND CONDUCTED A FEASIBILITY RANDOMIZED CONTROLLED TRIAL (RCT) IN INDIA. THE OBJECTIVE OF THIS STUDY WAS TO IDENTIFY AND EXPLORE WHY POTENTIAL PARTICIPANTS DECLINED TO PARTICIPATE IN THE FEASIBILITY RCT. METHODS: AN EXPLORATORY QUALITATIVE STUDY, USING SEMI-STRUCTURED INTERVIEWS, WAS CONDUCTED AT A YOGA CENTER IN NEW DELHI, INDIA. FOURTEEN PEOPLE (10 WOMEN AND FOUR MEN) WHO DECLINED TO PARTICIPATE IN THE FEASIBILITY RCT WERE INTERVIEWED, AND 13 OF THEM COMPLETED THE NON-PARTICIPANT QUESTIONNAIRE, WHICH CAPTURED THEIR SOCIO-DEMOGRAPHICS, DIETS, PHYSICAL ACTIVITIES, AND REASONS FOR DECLINING. RESULTS: THREE TYPES OF BARRIERS WERE IDENTIFIED AND EXPLORED WHICH PREVENTED PARTICIPATION IN THE FEASIBILITY RCT: (1) PERSONAL BARRIERS, SUCH AS LACK OF TIME, PERCEIVED SUFFICIENCY OF KNOWLEDGE, PREFERENCES ABOUT SELF-MANAGEMENT OF HEALTH, AND TRUST IN OTHER TRADITIONAL AND ALTERNATIVE THERAPIES; (2) CONTEXTUAL BARRIERS, SUCH AS SOCIAL INFLUENCES AND LACK OF AWARENESS ABOUT PREVENTIVE CARE; AND (3) STUDY-RELATED BARRIERS, SUCH AS LACK OF STUDY INFORMATION, POOR ACCESSIBILITY TO THE YOGA SITE, AND LACK OF TRUST IN THE STUDY METHODS AND INTERVENTION. CONCLUSIONS: WE IDENTIFIED AND EXPLORED PERSONAL, CONTEXTUAL, AND STUDY-RELATED BARRIERS TO PARTICIPATION IN A FEASIBILITY RCT IN INDIA. CARDIAC REHABILITATION (CR) AFTER MYOCARDIAL INFARCTION IS HIGHLY EFFECTIVE. IT IS UNAVAILABLE IN PUBLIC HOSPITALS IN INDIA DUE TO LIMITED RESOURCES. OUR OBJECTIVE WAS TO DEVELOP A SCALABLE MODEL OF CR FOR INDIA BASED ON YOGA, WHICH COULD ALSO APPEAL TO SOME GROUPS WITH LOW UPTAKE OF CR (E.G., ETHNIC MINORITIES, WOMEN, AND OLDER PEOPLE) GLOBALLY. THE INTERVENTION WAS DEVELOPED USING A STRUCTURED PROCESS. A LITERATURE REVIEW AND CONSULTATIONS WITH YOGA EXPERTS, CR EXPERTS, AND POSTMYOCARDIAL INFARCTION PATIENTS WERE CONDUCTED TO SYSTEMATICALLY IDENTIFY AND SHORTLIST APPROPRIATE YOGA EXERCISES AND POSTURES, BREATHING EXERCISES, MEDITATION AND RELAXATION PRACTICES, AND LIFESTYLE CHANGES, WHICH WERE INCORPORATED INTO A CONVENTIONAL CR FRAMEWORK. THE DRAFT INTERVENTION WAS FURTHER REFINED BASED ON THE FEEDBACK FROM AN INTERNAL STAKEHOLDER GROUP AND AN EXTERNAL PANEL OF INTERNATIONAL EXPERTS, BEFORE BEING PILOTED WITH YOGA INSTRUCTORS AND PATIENTS WITH MYOCARDIAL INFARCTION. A FOUR-PHASE YOGA-BASED CR (YOGA-CARE) PROGRAMME WAS DEVELOPED FOR DELIVERY BY A SINGLE YOGA INSTRUCTOR WITH BASIC TRAINING. THE PROGRAMME CONSISTS OF A TOTAL OF 13 INSTRUCTOR-LED SESSIONS (2 INDIVIDUAL AND 11 GROUP) OVER A 3-MONTH PERIOD. GROUP SESSIONS INCLUDE GUIDED PRACTICE OF YOGA EXERCISES AND POSTURES, BREATHING EXERCISES, AND MEDITATION AND RELAXATION PRACTICES, AND SUPPORT FOR THE LIFESTYLE CHANGE AND COPING THROUGH A MODERATED DISCUSSION. PATIENTS ARE ENCOURAGED TO SELF-PRACTICE DAILY AT HOME AND CONTINUE LONG-TERM WITH THE HELP OF A BOOKLET AND DIGITAL VIDEO DISC (DVD). FAMILY MEMBERS/CARERS ARE ENCOURAGED TO JOIN THROUGHOUT. IN CONCLUSION, A NOVEL YOGA-BASED CR PROGRAMME HAS BEEN DEVELOPED, WHICH PROMISES TO PROVIDE A SCALABLE CR SOLUTION FOR INDIA AND AN ALTERNATIVE CHOICE FOR CR GLOBALLY. IT IS CURRENTLY BEING EVALUATED IN A LARGE MULTICENTRE RANDOMISED CONTROLLED TRIAL ACROSS INDIA. 2019 16 2259 31 THE PRACTICABILITY AND RELEVANCE OF DEVELOPING A YOGA INTERVENTION FOR MENTAL HEALTH CONSUMERS: A QUALITATIVE STUDY. THE AIM OF THE STUDY WAS TO UNDERSTAND THE PERSPECTIVES OF MENTAL HEALTH CONSUMERS, STAFF, AND YOGA TEACHERS ON THE CHARACTERISTICS OF A YOGA-BASED INTERVENTION TO BE DEVELOPED FOR CONSUMERS WITH A LONG-TERM MENTAL HEALTH CONDITION, IN ADDITION TO ANXIETY AND/OR DEPRESSION. TWO FOCUS GROUPS WERE HELD WITH MENTAL HEALTH CONSUMERS (N = 8), TWO FOCUS GROUPS WITH MENTAL HEALTH STAFF (N = 13) FROM A METROPOLITAN COMMUNITY MENTAL HEALTH SETTING, AND FIVE ONE-ON-ONE INTERVIEWS WITH YOGA TEACHERS. PARTICIPANTS WERE ASKED ABOUT THE FEASIBILITY AND APPROPRIATENESS OF A RANGE OF YOGA PRACTICE FEATURES TO BE TESTED AS A NEWLY DEVELOPED YOGA-BASED INTERVENTION. THEMATIC ANALYSIS WAS USED TO IDENTIFY THEMES. FINDINGS INCLUDED THE FOLLOWING CONSUMER THEMES: MENTAL HEALTH UNDERSTANDING AND EXPERIENCE OF TEACHERS AND PARTICIPANTS IS IMPORTANT, CREATING A SAFE SPACE AND YOGA PRACTICE ELEMENTS. STAFF THEMES INCLUDED MENTAL HEALTH UNDERSTANDING AND EXPERIENCE OF TEACHERS IS IMPORTANT, ENVIRONMENTAL DESIGN, AND YOGA PRACTICE ELEMENTS. YOGA TEACHER THEMES INCLUDED ENVIRONMENTAL DESIGN AND YOGA PRACTICE ELEMENTS. MENTAL HEALTH UNDERSTANDING AND EXPERIENCE OF THE YOGA TEACHER, AND TRAUMA-INFORMED CARE WERE CONSISTENTLY EMPHASIZED BY CONSUMERS AND STAFF. TEACHERS FOCUSED LESS ON SPECIFIC MENTAL HEALTH CONSIDERATIONS, WHICH MAY BE REFLECTIVE OF A BROADER KNOWLEDGE GAP. RECOMMENDATIONS REGARDING YOGA PRACTICE ELEMENTS, INCLUDING PRE-INFORMATION, REGULAR AND CONSISTENT PRACTICE, MODIFICATIONS, BREATHWORK, MINDFULNESS AND GUIDED PRACTICE, AND ENVIRONMENTAL DESIGN FEATURES, WERE CONSISTENT WITH CURRENT TRAUMA-INFORMED YOGA RESEARCH. FINDINGS WILL BE USED TO GUIDE THE DEVELOPMENT OF A YOGA-BASED INTERVENTION FOR CONSUMERS WITH A LONG-TERM MENTAL HEALTH CONDITION, IN ADDITION TO ANXIETY AND/OR DEPRESSION. 2020 17 145 33 A QUALITATIVE EVALUATION OF STUDENT LEARNING AND SKILLS USE IN A SCHOOL-BASED MINDFULNESS AND YOGA PROGRAM. PREVIOUS STUDIES ON SCHOOL-BASED MINDFULNESS AND YOGA PROGRAMS HAVE FOCUSED PRIMARILY ON QUANTITATIVE MEASUREMENT OF PROGRAM OUTCOMES. THIS STUDY USED QUALITATIVE DATA TO INVESTIGATE PROGRAM CONTENT AND SKILLS THAT STUDENTS REMEMBERED AND APPLIED IN THEIR DAILY LIVES. DATA WERE GATHERED FOLLOWING A 16-WEEK MINDFULNESS AND YOGA INTERVENTION DELIVERED AT THREE URBAN SCHOOLS BY A COMMUNITY NON-PROFIT ORGANIZATION. THE OTHER FIVE THEMES WERE: 'FEMININE NURTURING SAFE SPACE', 'WATCHING AND LEARNING THE MOTHERING', 'BUILDING MENTAL HEALTH, WELL-BEING AND CONNECTIONS', THE "GROUP LIKE A ROCK AND A SEED' AND 'DIFFERENT FROM MAINSTREAM'. INNOVATIVE THERAPIES ARE NEEDED TO ADDRESS THE DIVERSE, MENTAL AND PHYSICAL CONSEQUENCES OF THESE CHANGES. TRAUMA CENTER-TRAUMA SENSITIVE YOGA (TC-TSY) HAS SHOWN PROMISING RESULTS IN CLINICAL TRIALS AMONG INDIVIDUALS EXPERIENCING POSTTRAUMATIC SYMPTOMATOLOGY. MATERIALS AND METHODS: USING A QUALITATIVE, DESCRIPTIVE DESIGN, WE EXAMINED THE IMPACT OF TC-TSY TRAINING ON THE PROFESSIONAL PRACTICE OF SEVEN PRACTITIONERS WHO WERE CERTIFIED YOGA INSTRUCTORS AND/OR REGISTERED PSYCHOLOGISTS. RESULTS: THE FINDINGS SUGGEST TC-TSY TRAINING ENRICHED THEIR PRACTICES, CHIEFLY THROUGH ITS CHOICE-BASED, WIDELY APPLICABLE APPROACH; INVITATION- AND MINDFULNESS-BASED LANGUAGE; AND BUILDING OF THERAPEUTIC TRUST AND ATTUNEMENT. SOME PARTICIPANTS QUESTIONED THE ACCESSIBILITY AND SUITABILITY OF THE TRAINING, AS WELL AS THE RIGIDITY OF TC-TSY TERMINOLOGY. CONCLUSION: MORE RESEARCH IS NEEDED TO EXAMINE POTENTIAL OUTCOMES OF TC-TSY PRACTICE RELATED TO SELF-EFFICACY, COPING, QUALITY OF LIFE, AND STRESS REDUCTION AMONG TRAUMATIZED INDIVIDUALS. 2021 20 1842 32 QUALITATIVE EVALUATION OF A COMMUNITY-BASED PHYSICAL ACTIVITY AND YOGA PROGRAM FOR MEN LIVING WITH PROSTATE CANCER: SURVIVOR PERSPECTIVES. PURPOSE: PHYSICAL ACTIVITY (PA) PROGRAMS FOR PROSTATE CANCER SURVIVORS HAVE POSITIVE EFFECTS ON MANY ASPECTS OF HEALTH-RELATED QUALITY OF LIFE. TRANSLATING THIS RESEARCH INTO SUSTAINABLE COMMUNITY-BASED SETTINGS IS NECESSARY TO ENSURE ACCESS TO PROGRAMS FOR SURVIVORS. THIS STUDY EXAMINES PATIENT PERSPECTIVES IN THE COMMUNITY-BASED TRUENTH LIFESTYLE MANAGEMENT (TRUENTH LM) PROGRAM IN CALGARY, CANADA. METHODS: ELEVEN MEN FROM PROGRAMS AT CIVIC WELLNESS CENTERS PARTICIPATED IN 2 SMALL SEMISTRUCTURED FOCUS GROUPS (N = 5 AND 6) AT THE UNIVERSITY OF CALGARY. MOTIVATION FOR PROGRAM INITIATION AND ADHERENCE, BENEFITS AND BARRIERS TO PARTICIPATION, AND INDIVIDUAL SATISFACTION AND FEEDBACK ON PROGRAM IMPROVEMENT WERE DISCUSSED. AUDIO RECORDINGS WERE TRANSCRIBED AND ANALYZED USING THEMATIC METHODOLOGY GUIDED BY A PRAGMATIC PHILOSOPHY ON THE PATIENT EXPERIENCE IN THE PROGRAM. RESULTS: THEMES IDENTIFIED INCLUDED PERCEIVED BENEFITS OF PARTICIPATING (PHYSICAL, PSYCHOLOGICAL, AND SOCIAL), FACILITATORS FOR INVOLVEMENT IN THE PA PROGRAM (PROGRAM DESIGN, INITIAL FREE ACCESS, TAILORED TO PROSTATE CANCER SPECIFIC NEEDS, PSYCHOSOCIAL ENVIRONMENT), AND OPPORTUNITIES FOR IMPROVEMENT AND SUSTAINABILITY (EXERCISE AS A PART OF STANDARD CARE, COST STRUCTURE, HOME-BASED OPTIONS). CONCLUSIONS: THESE FINDINGS PROVIDE VALUABLE INSIGHT INTO PATIENT PERSPECTIVES ON EFFECTIVE CHARACTERISTICS OF PROSTATE CANCER AND EXERCISE PROGRAMS. TRUENTH LM HAS IMPLEMENTED FINDINGS, AND ENSURING NEEDS (BENEFITS AND BARRIERS) ARE ADDRESSED FOR PROSTATE CANCER SURVIVORS WHEN ENTERING COMMUNITY-BASED PA PROGRAMS. 2019
