OBJECTIVE: IN AN 8-WEEK NONPHARMACOLOGICAL PAIN INTERVENTION TRIAL AMONG OLDER ADULTS WITH LOWER EXTREMITY OSTEOARTHRITIS (OA), WE AIMED TO EXAMINE: A) THE BASELINE FRAILTY LEVEL OF THE PARTICIPANTS; B) WHETHER SUCH INTERVENTION IS MORE BENEFICIAL FOR BASELINE FRAILER OLDER ADULTS THAN FOR THEIR COUNTERPARTS WITH LESS FRAILTY; AND C) WHETHER THE INTERVENTION COULD ALSO ALTER FRAILTY. METHODS: PARTICIPANTS WERE RANDOMLY ASSIGNED TO EITHER CHAIR YOGA (CY) OR HEALTH EDUCATION PROGRAM (HEP) GROUPS AND ATTENDED TWICE-WEEKLY 45-MINUTE CY OR HEP SESSIONS FOR 8 WEEKS. FOLLOWING A STANDARD PROCEDURE, 82 VARIABLES WERE USED TO CONSTRUCT A FRAILTY INDEX (FI, 0-1). PRIMARY OUTCOMES WERE: WESTERN ONTARIO AND MCMASTER UNIVERSITIES (WOMAC) PAIN AND PAIN INTERFERENCE. LINEAR MIXED-EFFECTS MODELS WERE USED TO EVALUATE THE MODIFYING EFFECT OF BASELINE FRAILTY ON THE INTERVENTION EFFECT OF CY ON PRIMARY OUTCOMES. SIMILAR MODELS WERE USED TO EVALUATE THE EFFECT OF CY ON FRAILTY. RESULTS: A TOTAL OF 112 PARTICIPANTS (N = 63 CY, N = 49 HEP; 75.3 [SD = 7.5] YEARS) WITH 85 FEMALES (75.9%) WERE INCLUDED. THE MEAN VALUES OF BASELINE FI FOR THE CY AND HEP GROUPS WERE SIMILAR (0.428 [0.05] AND 0.433 [0.05], P = 0.355). EACH 0.01 INCREMENT IN BASELINE FI WAS ASSOCIATED WITH HIGHER WOMAC PAIN (BETA = 0.28, P < 0.001) AND PAIN INTERFERENCE (BETA = 0.51, P < 0.001). THERE WAS A SIGNIFICANT INTERACTION EFFECT BETWEEN INTERVENTION, TIME, AND BASELINE FI (P = 0.020 FOR WOMAC PAIN; P = 0.010 FOR PAIN INTERFERENCE), INDICATING THAT PARTICIPANTS WITH HIGHER LEVEL OF BASELINE FI HAD GREATER DECLINES IN WOMAC PAIN AND PAIN INTERFERENCE. OBJECTIVE: THIS STUDY AIMS TO DETERMINE THE EFFICACY OF YOGA IN ALLEVIATING VASOMOTOR SYMPTOMS (VMS) FREQUENCY AND BOTHER. METHODS: THIS STUDY WAS A THREE-BY-TWO FACTORIAL, RANDOMIZED CONTROLLED TRIAL. ELIGIBLE WOMEN WERE RANDOMIZED TO YOGA (N = 107), EXERCISE (N = 106), OR USUAL ACTIVITY (N = 142), AND WERE SIMULTANEOUSLY RANDOMIZED TO A DOUBLE-BLIND COMPARISON OF OMEGA-3 FATTY ACID (N = 177) OR PLACEBO (N = 178) CAPSULES. YOGA INTERVENTION CONSISTED OF 12 WEEKLY 90-MINUTE YOGA CLASSES WITH DAILY HOME PRACTICE. PRIMARY OUTCOMES WERE VMS FREQUENCY AND BOTHER ASSESSED BY DAILY DIARIES AT BASELINE, 6 WEEKS, AND 12 WEEKS. DESIGN: QUASI-EXPERIMENTAL STUDY WITH A PRETEST/POST-TEST DESIGN. RESEARCHERS EVALUATED CHANGES OVER TIME (PRETEST TO POST-TEST) IN ALL OUTCOME MEASURES. ALTHOUGH CHAIR YOGA WAS EFFECTIVE IN IMPROVING PHYSICAL FUNCTION AND REDUCING STIFFNESS IN OLDER ADULTS WITH OSTEOARTHRITIS, IT WAS NOT EFFECTIVE IN REDUCING PAIN LEVEL OR IMPROVING DEPRESSIVE SYMPTOMS. FUTURE RESEARCH PLANNED BY THIS TEAM WILL USE RIGOROUS STUDY METHODS, INCLUDING LARGER SAMPLES, RANDOMIZED CONTROLLED TRIALS, AND FOLLOW UP FOR MONITORING HOME PRACTICE AFTER THE INTERVENTIONS. 2012 19 1007 51 EFFECTS OF MINDFULNESS YOGA VS STRETCHING AND RESISTANCE TRAINING EXERCISES ON ANXIETY AND DEPRESSION FOR PEOPLE WITH PARKINSON DISEASE: A RANDOMIZED CLINICAL TRIAL. IMPORTANCE: CLINICAL PRACTICE GUIDELINES SUPPORT EXERCISE FOR PATIENTS WITH PARKINSON DISEASE (PD), BUT TO OUR KNOWLEDGE, NO RANDOMIZED CLINICAL TRIALS HAVE TESTED WHETHER YOGA IS SUPERIOR TO CONVENTIONAL PHYSICAL EXERCISES FOR STRESS AND SYMPTOM MANAGEMENT. OBJECTIVE: TO COMPARE THE EFFECTS OF A MINDFULNESS YOGA PROGRAM VS STRETCHING AND RESISTANCE TRAINING EXERCISE (SRTE) ON PSYCHOLOGICAL DISTRESS, PHYSICAL HEALTH, SPIRITUAL WELL-BEING, AND HEALTH-RELATED QUALITY OF LIFE (HRQOL) IN PATIENTS WITH MILD-TO-MODERATE PD. DESIGN, SETTING, AND PARTICIPANTS: AN ASSESSOR-MASKED, RANDOMIZED CLINICAL TRIAL USING THE INTENTION-TO-TREAT PRINCIPLE WAS CONDUCTED AT 4 COMMUNITY REHABILITATION CENTERS IN HONG KONG BETWEEN DECEMBER 1, 2016, AND MAY 31, 2017. A TOTAL OF 187 ADULTS (AGED >/=18 YEARS) WITH A CLINICAL DIAGNOSIS OF IDIOPATHIC PD WHO WERE ABLE TO STAND UNAIDED AND WALK WITH OR WITHOUT AN ASSISTIVE DEVICE WERE ENROLLED VIA CONVENIENCE SAMPLING. ELIGIBLE PARTICIPANTS WERE RANDOMIZED 1:1 TO MINDFULNESS YOGA OR SRTE. INTERVENTIONS: MINDFULNESS YOGA WAS DELIVERED IN 90-MINUTE GROUPS AND SRTE WERE DELIVERED IN 60-MINUTE GROUPS FOR 8 WEEKS. MAIN OUTCOMES AND MEASURES: PRIMARY OUTCOMES INCLUDED ANXIETY AND DEPRESSIVE SYMPTOMS ASSESSED USING THE HOSPITAL ANXIETY AND DEPRESSION SCALE. SECONDARY OUTCOMES INCLUDED SEVERITY OF MOTOR SYMPTOMS (MOVEMENT DISORDER SOCIETY UNIFIED PARKINSON'S DISEASE RATING SCALE [MDS-UPDRS], PART III MOTOR SCORE), MOBILITY, SPIRITUAL WELL-BEING IN TERMS OF PERCEIVED HARDSHIP AND EQUANIMITY, AND HRQOL. ASSESSMENTS WERE DONE AT BASELINE, 8 WEEKS (T1), AND 20 WEEKS (T2). RESULTS: THE 138 PARTICIPANTS INCLUDED 65 MEN (47.1%) WITH A MEAN (SD) AGE OF 63.7 (8.7) YEARS AND A MEAN (SD) MDS-UPDRS SCORE OF 33.3 (15.3). GENERALIZED ESTIMATING EQUATION ANALYSES REVEALED THAT THE YOGA GROUP HAD SIGNIFICANTLY BETTER IMPROVEMENT IN OUTCOMES THAN THE SRTE GROUP, PARTICULARLY FOR ANXIETY (TIME-BY-GROUP INTERACTION, T1: BETA, -1.79 [95% CI, -2.85 TO -0.69; P = .001]; T2: BETA, -2.05 [95% CI, -3.02 TO -1.08; P < .001]), DEPRESSION (T1: BETA, -2.75 [95% CI, -3.17 TO -1.35; P < .001]); T2: BETA, -2.75 [95% CI, -3.71 TO -1.79; P < .001]), PERCEIVED HARDSHIP (T1: BETA, -0.92 [95% CI, -1.25 TO -0.61; P < .001]; T2: BETA, -0.76 [95% CI, -1.12 TO -0.40; P < .001]), PERCEIVED EQUANIMITY (T1: BETA, 1.11 [95% CI, 0.79-1.42; P < .001]; T2: BETA, 1.19 [95% CI, 0.82-1.56; P < .001]), AND DISEASE-SPECIFIC HRQOL (T1: BETA, -7.77 [95% CI, -11.61 TO -4.38; P < .001]; T2: BETA, -7.99 [95% CI, -11.61 TO -4.38; P < .001]). CONCLUSIONS AND RELEVANCE: AMONG PATIENTS WITH MILD-TO-MODERATE PD, THE MINDFULNESS YOGA PROGRAM WAS FOUND TO BE AS EFFECTIVE AS SRTE IN IMPROVING MOTOR DYSFUNCTION AND MOBILITY, WITH THE ADDITIONAL BENEFITS OF A REDUCTION IN ANXIETY AND DEPRESSIVE SYMPTOMS AND AN INCREASE IN SPIRITUAL WELL-BEING AND HRQOL. OBJECTIVE: THE PURPOSE OF THIS PRAGMATIC CONTROLLED TRIAL WAS TO EXAMINE CHANGES IN PSYCHOLOGICAL AND OCCUPATIONAL WELL-BEING IN PROFESSIONALS WHO ATTENDED A YOGA-BASED PROGRAM. SETTING: THE 5-DAY RISE (RESILIENCE, INTEGRATION, SELF-AWARENESS, ENGAGEMENT) PROGRAM WAS DELIVERED AT THE KRIPALU CENTER FOR YOGA & HEALTH. RISE INCLUDED 5 H PER DAY OF YOGA, MEDITATION, LECTURES, AND EXPERIENTIAL ACTIVITIES. SUBJECTS: ADULT PROFESSIONALS FROM EDUCATION, CORRECTIONS, AND SOCIAL SERVICE INSTITUTIONS WERE PRAGMATICALLY ASSIGNED TO THE RISE GROUP (N = 61) OR A WAITLIST CONTROL GROUP (N = 60). OUTCOME MEASURES: MEASURES OF PSYCHOLOGICAL AND OCCUPATIONAL WELL-BEING WERE COMPLETED BEFORE RISE (BASELINE), IMMEDIATELY AFTER RISE (POSTPROGRAM), AND 2 MONTHS AFTER RISE (FOLLOW-UP). ANALYSES OF COVARIANCE WERE CONDUCTED TO COMPARE CHANGE SCORES BETWEEN GROUPS. RESULTS: EIGHTY-TWO PARTICIPANTS (RISE N = 41, CONTROL N = 41) COMPLETED BASELINE AND POSTMEASURES AND WERE INCLUDED IN THE ANALYSIS, AND 57 (RISE N = 27, CONTROL N = 30) ALSO COMPLETED THE FOLLOW-UP. RELATIVE TO CONTROLS, THE RISE GROUP REPORTED IMPROVEMENTS IN STRESS (P = 0.001, R(2) = 0.51), RESILIENCE (P = 0.028, R(2) = 0.34), POSITIVE AFFECT (P = 0.001, R(2) = 0.52), NEGATIVE AFFECT (P = 0.001, R(2) = 0.52), MINDFULNESS (P = 0.021, R(2) = 0.13), AND JOB SATISFACTION (P = 0.034, R(2) = 0.08) FROM BASELINE TO POSTPROGRAM. FROM BASELINE TO FOLLOW-UP, COMPARED WITH CONTROLS THE RISE GROUP SHOWED IMPROVEMENTS IN STRESS (P = 0.001, R(2) = 0.33), RESILIENCE (P = 0.001, R(2) = 0.24), POSITIVE AFFECT (P = 0.006, R(2) = 0.49), NEGATIVE AFFECT (P = 0.043, R(2) = 0.32), MINDFULNESS (P = 0.001, R(2) = 0.28), EMPOWERMENT (P = 0.005, R(2) = 0.20), AND SELF-COMPASSION (P = 0.011, R(2) = 0.19). CONCLUSIONS: THE RISE PROGRAM WAS ASSOCIATED WITH IMPROVEMENTS IN PSYCHOLOGICAL AND OCCUPATIONAL WELL-BEING IMMEDIATELY AFTER AND 2 MONTHS AFTER THE PROGRAM. FUTURE RESEARCH IS NEEDED TO CONFIRM THESE RESULTS. 2019
