1 265 122 ACUTE EFFECTS OF AEROBIC EXERCISE AND HATHA YOGA ON CRAVING TO SMOKE. INTRODUCTION: RECENT STUDIES HAVE EXAMINED THE EFFECTS OF PHYSICAL ACTIVITY ON CRAVING TO SMOKE AND SMOKING WITHDRAWAL. THE CURRENT STUDY WAS DESIGNED TO COMPARE AND CONTRAST THE EFFECTS OF 2 DIFFERENT FORMS OF PHYSICAL ACTIVITY ON GENERAL AND CUE-ELICITED CRAVING TO SMOKE. METHODS: FOLLOWING 1-HR NICOTINE ABSTINENCE, 76 DAILY SMOKERS WERE RANDOMLY ASSIGNED TO ENGAGE IN A 30-MIN BOUT OF CARDIOVASCULAR EXERCISE (CE; BRISK WALK ON A TREADMILL), HATHA YOGA (HY), OR A NONACTIVITY CONTROL CONDITION. PARTICIPANTS COMPLETED MEASURES OF CRAVING AND MOOD, AND A SMOKING CUE REACTIVITY ASSESSMENT, BEFORE, IMMEDIATELY FOLLOWING, AND APPROXIMATELY 20 MIN AFTER THE PHYSICAL ACTIVITY OR CONTROL CONDITIONS. RESULTS: COMPARED WITH THE CONTROL CONDITION, PARTICIPANTS IN EACH OF THE PHYSICAL ACTIVITY GROUPS REPORTED A DECREASE IN CRAVING TO SMOKE, AN INCREASE IN POSITIVE AFFECT, AND A DECREASE IN NEGATIVE AFFECT. IN ADDITION, CRAVING IN RESPONSE TO SMOKING CUES WAS SPECIFICALLY REDUCED AMONG THOSE WHO ENGAGED IN CE, WHEREAS THOSE WHO ENGAGED IN HY REPORTED A GENERAL DECREASE IN CRAVINGS. CONCLUSIONS: THIS STUDY PROVIDES FURTHER SUPPORT FOR THE USE OF EXERCISE BOUTS FOR ATTENUATING CIGARETTE CRAVINGS DURING TEMPORARY NICOTINE ABSTINENCE. THERE IS INCREASING INTEREST IN THE ADJUNCTIVE USE OF MINDFULNESS-BASED INTERVENTIONS, SUCH AS YOGA, TO IMPROVE TREATMENT OUTCOMES. THE CURRENT STUDY WAS A PRELIMINARY NATURALISTIC INVESTIGATION OF A NOVEL TRAUMA-INFORMED YOGA INTERVENTION IN AN INPATIENT TREATMENT PROGRAM FOR WOMEN WITH SUBSTANCE USE DISORDER (SUD). CHANGES AND DIFFERENCES IN SOMATIC SYMPTOMS, PSYCHIATRIC SYMPTOMS, AND PSYCHOLOGICAL MECHANISMS WERE EVALUATED IN WOMEN RECEIVING TREATMENT-AS-USUAL (N = 36) AND TREATMENT-AS-USUAL PLUS THE YOGA INTERVENTION (N = 42). FOR BOTH GROUPS, STATISTICALLY SIGNIFICANT WITHIN-SUBJECTS CHANGES WERE PRESENT FOR SOMATIC AND PSYCHIATRIC SYMPTOMS, CRAVINGS, SELF-EFFICACY, AND MULTIPLE FACETS OF IMPULSIVITY AND MINDFULNESS. DESIGN: THE RESEARCH TEAM FIRST PERFORMED A LITERATURE REVIEW OF TRADITIONAL AND CONTEMPORARY YOGA TEXTS, SUCH AS HATHA YOGA PRADIPIKA AND LIGHT ON YOGA, AS WELL AS MODERN SCIENTIFIC LITERATURE IN THE FOLLOWING SEARCH ENGINES-GOOGLE SCHOLAR, PUBMED, AND PSYCHINFO, USING THE KEYWORDS YOGA, PRANAYAMA, HATHA YOGA, RELAXATION. MEDITATION, SUBSTANCE USE, ADDICTION, IMPULSIVITY, CRAVING, SLEEP QUALITY, AND FATIGUE. USING THE INFORMATION OBTAINED, THE TEAM DEVELOPED A YOGA PROGRAM AND DESIGNED A PILOT STUDY THAT USED THE PROGRAM. SETTING: THE STUDY TOOK PLACE IN THE DEPARTMENT OF INTEGRATIVE MEDICINE AT THE NATIONAL INSTITUTE OF MENTAL HEALTH AND NEUROSCIENCES (NIMHANS) IN BANGALORE, INDIA. OUTCOME MEASURES: FOR VALIDATION, 13 EXPERTS SCORED THE YOGA PROGRAM THAT THE RESEARCH TEAM HAD DEVELOPED AND GAVE SUGGESTIONS FOR EACH YOGIC PRACTICE FOR USE DURING THE ACUTE PHASE OF WITHDRAWAL AND THE MAINTENANCE PHASE RESPECTIVELY. A CONTENT VALIDITY RATIO (CVR) WAS CALCULATED FROM THEIR SCORING, AND THE RESEARCH TEAM MADE CHANGES TO THE PROGRAM BASE ON THE SCORING AND SUGGESTIONS. FOR THE PILOT STUDY, ASSESSMENTS OCCURRED AT BASELINE AND POSTINTERVENTION. THE PARTICIPANTS' YOGA PERFORMANCE WAS RATED BY THE YOGA TRAINER ON A YOGA PERFORMANCE ASSESSMENT SCALE (YPA). OTHER MEASUREMENTS INCLUDED: (1) THE CLINICAL OPIATE WITHDRAWAL SCALE (COWS), (2) THE HAMILTON'S ANXIETY RATING SCALE (HAM-A), (3) THE HAMILTON'S DEPRESSION RATING SCALE (HAM-D), (4) BUPRENORPHINE DOSAGE, (5) THE CLINICAL GLOBAL IMPRESSION SEVERITY (CGI-S) SCALE, (6) A VISUAL ANALOG SCALE (VAS) FOR PAIN, (7) SLEEP QUALITY (LATENCY AND DURATION), AND (8) THE MODULE'S SAFETY. RESULTS: FOUR PRACTICES WERE REMOVED FROM THE PROGRAM DUE TO CVR SCORES BELOW THE CUTOFF, AND ONE PRACTICE WAS FOUND NOT TO BE FEASIBLE (KAPALABHATI). TWO CATEGORIES OF YOGA MODULES EMERGED: (1) FOR THE ACUTE SYMPTOMATIC PHASE (40 MINUTES) AND (2) FOR THE MAINTENANCE PHASE (ONE HOUR). PRACTICES WERE ADDED OR EXCLUDED BASED ON THE PHASE. CONCLUSIONS: RESULTS FROM THIS STUDY ADD TO A GROWING BODY OF RESEARCH DEMONSTRATING THE POTENTIAL CLINICAL UTILITY OF HATHA YOGA AS AN ADJUNCTIVE INTERVENTION TOOL FOR SMOKING CESSATION. CURRENT THEORETICAL MODELS SUGGEST THAT THE SKILLS, INSIGHTS, AND SELF-AWARENESS LEARNED THROUGH YOGA AND MINDFULNESS PRACTICE CAN TARGET MULTIPLE PSYCHOLOGICAL, NEURAL, PHYSIOLOGICAL, AND BEHAVIORAL PROCESSES IMPLICATED IN ADDICTION AND RELAPSE. WE AIMED TO DETERMINE HOW AN ACUTE SESSION OF YOGA (AND ITS SPECIFIC COMPONENTS) RELATED TO PRE- TO POST- SESSION CHANGES IN PROPOSED MECHANISMS (PSYCHOLOGICAL RESOURCES) AND WHETHER THOSE CHANGES WERE ASSOCIATED WITH POSITIVE CHANGES IN EMOTIONS. METHODS/DESIGN: THE BREATHEASY STUDY IS A RIGOROUS, RANDOMIZED CONTROLLED CLINICAL TRIAL EXAMINING THE EFFICACY OF IYENGAR YOGA AS A COMPLEMENTARY THERAPY TO COGNITIVE-BEHAVIORAL THERAPY FOR SMOKING CESSATION. RESULTS: OVERALL, THERAPEUTIC YOGA PROGRAMS ARE FREQUENTLY EFFECTIVE IN PROMOTING WEIGHT LOSS AND/OR IMPROVEMENTS IN BODY COMPOSITION. EVIDENCE SUPPORTS THE USE OF YOGA FOR SUD; HOWEVER FEW STUDIES SPECIFICALLY FEATURE WOMEN. OBJECTIVES: COMMUNITY-BASED YOGA MAY ADD TO HEALTH PROMOTION THROUGH PREFERABLE PHYSICAL ACTIVITY FOR WOMEN IN RECOVERY. THE AIMS OF THIS STUDY ARE TO EXPLORE DEMOGRAPHICS AND QUANTITATIVE MEASURES RELEVANT TO RECOVERY AND CAPTURE AND UNDERSTAND THE SUBJECTIVE EXPERIENCE OF ONE SESSION OF YOGA. STUDY DESIGN: THE STUDY DESIGN INVOLVES DESCRIPTIVE/CROSS-SECTIONAL. METHODOLOGY: WOMEN IN AN INPATIENT SUD CENTER ATTENDING WEEKLY OPTIONAL OFF-SITE YOGA FOR RECOVERY WERE RECRUITED TO CAPTURE FIRST-TIME ATTENDANCE. SURVEY DATA INCLUDED MEDICAL OUTCOMES SURVEY 12-ITEM SHORT-FORM (SF-12), TORONTO MINDFULNESS SCALE (TMS), AND BRIEF RESILIENCE SCALE (BRS), DEMOGRAPHICS, AND NARRATIVE REFLECTIONS. SUBOPTIMAL SF-12 SCORES WERE FOUND IN WOMEN WITH SUD AND, THEREFORE IMPORTANT TO NOTE IN THE CONTEXT OF RECOVERY TO OPTIMIZE TREATMENT. YOGA NOT THUS ADAPTED, ON THE OTHER HAND, MAY INCREASE REACTIVITY AND ACTIVATE SYMPTOMS SUCH AS HYPERAROUSAL OR DISSOCIATION. FINDINGS REVEALED THAT TIY NEEDS TO EMPHASIZE BENEFICIAL PRACTICES (E.G., DIAPHRAGMATIC BREATH AND RESTORATIVE POSTURES), CONSIDER CONTRAINDICATIONS (E.G., AVOIDING SEQUENCES THAT OVERLY ENGAGE THE SYMPATHETIC NERVOUS SYSTEM), ADAPT TO LIMITATIONS AND CHALLENGES FOR TEACHING IN UNCONVENTIONAL SETTINGS (E.G., PRISONS, VA HOSPITALS), AND PROVIDE SPECIALIZED TRAINING AND PREPARATION (E.G., SPECIALIZED TIY CERTIFICATIONS, SELF-CARE OF INSTRUCTORS/THERAPISTS, ADAPTIONS FOR STUDENT NEEDS). WHILE PUTATIVE MECHANISMS BEHIND YOGA'S IMPACT ON SUBSTANCE USE HAVE BEEN PROPOSED, THE RESEARCH IS LIMITED. THIS MANUSCRIPT AIMS TO DETERMINE HOW A HATHA YOGA INTERVENTION IMPACTS THE SUBSTANCE USE OF PEOPLE WHO ARE IN REENTRY FROM PRISON OR JAIL (RETURNING CITIZENS), AND LIVING WITH HIV AND SUBSTANCE USE PROBLEMS. DESIGN: SEMI-STRUCTURED INTERVIEWS WERE CONDUCTED WITH 28 RETURNING CITIZENS WHO PARTICIPATED IN A YOGA INTERVENTION. GUIDED BY A QUALITATIVE DESCRIPTIVE METHODOLOGY, THEMATIC ANALYSIS WAS USED TO IDENTIFY THEMES THAT ANSWERED HOW YOGA IMPACTED PARTICIPANT SUBSTANCE USE. SETTING: A COMMUNITY ORGANIZATION IN PHILADELPHIA, PA, USA THAT CONNECTS PEOPLE TO HEALTH SERVICES, PROVIDES EDUCATION AND SUPPORTIVE SERVICES, AND ADVOCATES FOR PEOPLE WITH CRIMINAL JUSTICE INVOLVEMENT. INTERVENTION: A 90-MINUTE HATHA YOGA CLASS OFFERED ONCE A WEEK FOR 12 WEEKS. RESULTS: FOURTEEN PARTICIPANTS REPORTED THAT YOGA EITHER REDUCED SUBSTANCE USE OR MAINTAINED NON-USE, VIA THE MECHANISMS OF PURPOSEFUL DISTRACTION, STRESS COPING (BY CULTIVATING MINDFULNESS AND REDUCING PHYSICAL DISCOMFORT), SOCIAL SUPPORT AND CONFIDENCE. ELEVEN PARTICIPANTS REPORTED THAT YOGA DID NOT IMPACT THEIR SUBSTANCE USE. THREE PARTICIPANTS DID NOT DISCUSS IT. CONCLUSIONS: BY PROVIDING PURPOSEFUL DISTRACTION, INCREASED STRESS COPING, SOCIAL SUPPORT AND CONFIDENCE; YOGA MAY REDUCE SUBSTANCE USE AND MAINTAIN ENGAGEMENT IN RECOVERY. THESE MECHANISTIC ACTIONS PROVIDE GUIDANCE FOR THEMES TO HIGHLIGHT IN YOGA CLASSES THAT AIM TO IMPACT SUBSTANCE USE AMONG RETURNING CITIZENS LIVING WITH HIV. 2019
