IN THIS SETTING, MYELOPATHY THROUGH FURTHER FLATTENING OF THE CORD FROM EXTRINSIC COMPRESSION CAN BE PRECIPITATED BY RELATIVELY MINOR TRAUMAS. ARTERIAL DISSECTION IS SIMILARLY CONSIDERED A RESULT OF HIGH VELOCITY OR MOMENTUM DURING TRAUMA, COMMONLY ASSOCIATED WITH FRACTURES, CERVICAL HYPERFLEXION, OR DIRECT BLUNT FORCE TO THE NECK. OVERALL, PRECAUTIONS FOR BOTH ARTERIAL DISSECTION AND MYELOPATHY ARE RARELY CONSIDERED IN LOW-VELOCITY, STATIC ACTIVITIES SUCH AS YOGA. CASE PRESENTATION: THE AUTHORS REPORT THE CASE OF A 63-YEAR-OLD MAN WHO SUFFERED CONCURRENT CERVICAL MYELOPATHY FROM MULTILEVEL SPONDYLOPATHY, RIGHT VERTEBRAL ARTERY DISSECTION, AND LEFT CERVICAL CAROTID ARTERY DISSECTION FOLLOWING A YOGA SESSION. SYMPTOMATOLOGY CONSISTED OF ACUTE ONSET NECK PAIN, UPPER EXTREMITY SENSORY PARESTHESIA, WORSENING GAIT AND BALANCE, AND IMPAIRED DEXTERITY FOR SEVERAL WEEKS. CERVICAL MRI WAS OBTAINED GIVEN MYELOPATHIC SYMPTOMS AND REVEALED SPONDYLOSIS WITH COMPRESSION AND T2 SIGNAL CHANGE AT C3-C4. CT ANGIOGRAPHY OF THE NECK REVEALED AFOREMENTIONED DISSECTIONS WITHOUT FLOW LIMITING STENOSIS OR OCCLUSION. A THERAPEUTIC HEPARIN INFUSION WAS STARTED PREOPERATIVELY UNTIL THE PATIENT UNDERWENT C3-C4 ANTERIOR CERVICAL DISCECTOMY AND FUSION. ASPIRIN AND PLAVIX WERE THEN STARTED WITHOUT INCIDENCE AND THE PATIENT HAD SIGNIFICANT BUT GRADUAL IMPROVEMENT IN MYELOPATHIC SYMPTOMS AT 6-WEEK FOLLOW-UP. THERE IS GROWING EVIDENCE FOR THE EFFECTIVENESS OF YOGA FOR RELIEVING MUSCULOSKELETAL DISORDERS. HOWEVER, DATA ON THE EFFECTS AND COMPARISON OF THESE SPECIFIC EXERCISES ON THE CERVICAL MUSCLE MORPHOLOGY ARE INSUFFICIENT OR LACKING. THE OBJECTIVE OF THE CURRENT STUDY WAS TO FIND OUT WHETHER YOGA PRACTICE WAS BENEFICIAL TO THE SPINE BY COMPARING DEGENERATIVE DISC DISEASE IN THE SPINES OF LONG-TIME YOGA PRACTITIONERS AND NON-YOGA PRACTICING CONTROLS, USING AN OBJECTIVE MEASUREMENT TOOL, MAGNETIC RESONANCE IMAGING. THIS MATCHED CASE-CONTROL STUDY COMPRISED 18 YOGA INSTRUCTORS WITH TEACHING EXPERIENCE OF MORE THAN 10 YEARS AND 18 NON-YOGA PRACTICING ASYMPTOMATIC INDIVIDUALS RANDOMLY SELECTED FROM A HEALTH CHECKUP DATABASE. A VALIDATED GRADING SCALE WAS USED TO GRADE THE CONDITION OF CERVICAL AND LUMBAR DISCS SEEN IN MAGNETIC RESONANCE IMAGING OF THE SPINE, AND THE RESULTING DATA ANALYZED STATISTICALLY. THE MEAN NUMBER OF YEARS OF YOGA PRACTICE FOR THE YOGA GROUP WAS 12.9 +/- 7.5. THE OVERALL (CERVICAL + LUMBAR) DISC SCORES OF THE YOGA GROUP WERE SIGNIFICANTLY LOWER (INDICATING LESS DEGENERATIVE DISC DISEASE) THAN THOSE OF THE CONTROL GROUP (P < 0.001). THE SCORES FOR THE CERVICAL VERTEBRAL DISCS OF THE YOGA GROUP WERE ALSO SIGNIFICANTLY LOWER THAN THOSE OF THE CONTROL GROUP (P < 0.001), WHILE THE LOWER SCORES FOR THE YOGA GROUP IN THE LUMBAR GROUP APPROACHED, BUT DID NOT REACH, STATISTICAL SIGNIFICANCE (P = 0.055). THE SCORES FOR INDIVIDUAL DISCS OF YOGA PRACTITIONERS SHOWED SIGNIFICANTLY LESS DEGENERATIVE DISEASE AT THREE DISC LEVELS, C3/C4, L2/L3 AND L3/L4 (P < 0.05). MAGNETIC RESONANCE IMAGING SHOWED THAT THE GROUP OF LONG-TERM PRACTITIONERS OF YOGA STUDIED HAD SIGNIFICANTLY LESS DEGENERATIVE DISC DISEASE THAN A MATCHED CONTROL GROUP. 2011 9 507 25 COMPARATIVE EFFECTIVENESS OF PILATES AND YOGA GROUP EXERCISE INTERVENTIONS FOR CHRONIC MECHANICAL NECK PAIN: QUASI-RANDOMISED PARALLEL CONTROLLED STUDY. PAIN RATINGS ALSO IMPROVED SIGNIFICANTLY. MODERATE-TO-LARGE EFFECT SIZES (0.7 TO 1.8) AND LOW NUMBERS NEEDED TO TREAT WERE FOUND. THERE WERE NO DIFFERENCES IN OUTCOMES BETWEEN THE EXERCISE GROUPS OR ASSOCIATED ADVERSE EFFECTS. NO IMPROVEMENTS IN RANGE OF MOVEMENT OR POSTURE WERE FOUND. THIS SYSTEMATIC REVIEW FOUND STRONG EVIDENCE FOR SHORT-TERM EFFECTIVENESS, BUT LITTLE EVIDENCE FOR LONG-TERM EFFECTIVENESS OF YOGA FOR CHRONIC SPINE PAIN IN THE PATIENT-CENTERED OUTCOMES. 2015 11 2777 31 YOGA SPINAL FLEXION POSITIONS AND VERTEBRAL COMPRESSION FRACTURE IN OSTEOPENIA OR OSTEOPOROSIS OF SPINE: CASE SERIES. OBJECTIVE: THE OBJECTIVE OF THIS REPORT IS TO RAISE AWARENESS OF THE EFFECT OF STRENUOUS YOGA FLEXION EXERCISES ON OSTEOPENIC OR OSTEOPOROTIC SPINES. WE PREVIOUSLY DESCRIBED SUBJECTS WITH KNOWN OSTEOPOROSIS IN WHOM VERTEBRAL COMPRESSION FRACTURES (VCFS) DEVELOPED AFTER SPINAL FLEXION EXERCISE (SFE) AND RECOMMENDED THAT SFES NOT BE PRESCRIBED IN PATIENTS WITH SPINAL OSTEOPOROSIS. METHODS: THIS REPORT DESCRIBES 3 HEALTHY PERSONS WITH LOW BONE MASS AND YOGA-INDUCED PAIN OR FRACTURE. RESULTS: ALL 3 PATIENTS HAD OSTEOPENIA, WERE IN GOOD HEALTH AND PAIN-FREE, AND HAD STARTED YOGA EXERCISES TO IMPROVE THEIR MUSCULOSKELETAL HEALTH. NEW PAIN AND FRACTURE AREAS OCCURRED AFTER PARTICIPATION IN YOGA FLEXION EXERCISES. CONCLUSIONS: THE DEVELOPMENT OF PAIN AND COMPLICATIONS WITH SOME FLEXION YOGA POSITIONS IN THE PATIENTS WITH OSTEOPENIA LEADS TO CONCERN THAT FRACTURE RISK WOULD INCREASE EVEN FURTHER IN OSTEOPOROSIS. THIS FINDING SUGGESTS THAT FACTORS OTHER THAN BONE MASS SHOULD BE CONSIDERED FOR EXERCISE COUNSELING IN PATIENTS WITH BONE LOSS. THE INCREASED TORQUE PRESSURE APPLIED TO VERTEBRAL BODIES DURING SFES MAY BE A RISK. SECONDARY OUTCOME MEASURES INCLUDED QUALITY OF LIFE, MOOD, AND SAFETY. RISK OF BIAS WAS ASSESSED USING THE COCHRANE TOOL. RESULTS: THREE STUDIES ON 188 PATIENTS WITH CHRONIC NON-SPECIFIC NECK PAIN COMPARING YOGA TO USUAL CARE WERE INCLUDED. METHODS: PUBMED, EMBASE, PSYCINFO, WEB OF SCIENCE, AND THE COCHRANE LIBRARY DATABASES WERE SEARCHED FROM INCEPTION TO FEBRUARY 9, 2019, AND SIX RANDOMIZED CONTROLLED TRIALS WERE SELECTED FOR THIS META-ANALYSIS. RESULTS: THE POOLED FIXED-EFFECT SIZE OF SIX TRIALS SHOWED THAT 12 WEEKS OF YOGA PROGRAMS COULD SIGNIFICANTLY REDUCE CHRONIC NONSPECIFIC LOWER BACK PAIN BY 0.41 WITHIN THE TRIALS (STANDARDIZED MEAN DIFFERENCE; 95% CONFIDENCE INTERVAL: -0.58 TO -0.23; P < .0001). SUBGROUP ANALYSES ALSO SHOWED THAT SIGNIFICANT PAIN REDUCTION WAS RELATED TO TYPE OF YOGA, LENGTH OF SESSION, STUDY QUALITY, AND TIMING OF PAIN ASSESSMENT. CONCLUSIONS: THESE FINDINGS REVEAL THAT 12 WEEKS OF YOGA CAN HELP ALLEVIATE PAIN, AND YOGA PROGRAMS SHOULD TAKE INTO ACCOUNT THE SUBGROUP FACTORS TO INCREASE INDIVIDUALS' RELIEF FROM CHRONIC NONSPECIFIC LOWER BACK PAIN. 2020 16 721 35 EFFECT OF IYENGAR YOGA THERAPY FOR CHRONIC LOW BACK PAIN. SECONDARY OUTCOMES INCLUDING PRESENT PAIN INTENSITY, PAIN MEDICATION USAGE, PAIN-RELATED ATTITUDES AND BEHAVIORS, AND SPINAL RANGE OF MOTION WERE MEASURED BEFORE AND AFTER THE INTERVENTIONS. SUBJECTS HAD LOW BACK PAIN FOR 11.2+/-1.54 YEARS AND 48% USED PAIN MEDICATION. OVERALL, SUBJECTS PRESENTED WITH LESS PAIN AND LOWER FUNCTIONAL DISABILITY THAN SUBJECTS IN OTHER PUBLISHED INTERVENTION STUDIES FOR CHRONIC LOW BACK PAIN. OF THE 60 SUBJECTS ENROLLED, 42 (70%) COMPLETED THE STUDY. MULTIVARIATE ANALYSES OF OUTCOMES IN THE CATEGORIES OF MEDICAL, FUNCTIONAL, PSYCHOLOGICAL AND BEHAVIORAL FACTORS INDICATED THAT SIGNIFICANT DIFFERENCES BETWEEN GROUPS EXISTED IN FUNCTIONAL AND MEDICAL OUTCOMES BUT NOT FOR THE PSYCHOLOGICAL OR BEHAVIORAL OUTCOMES. MORE COMPREHENSIVE DATA ARE NEEDED TO PROVIDE CLINICALLY SIGNIFICANT EVIDENCE FOR CLINICIANS TO CONFIDENTLY PRESCRIBE EXERCISES TO PATIENTS. THIS STUDY WAS DONE TO EVALUATE THE EFFECT OF ANTENATAL EXERCISES, INCLUDING YOGA, ON THE COURSE OF LABOR, DELIVERY, AND PREGNANCY OUTCOMES. METHODS: A RETROSPECTIVE STUDY WAS CONDUCTED AMONG 200 PRIMIPAROUS SUBJECTS (AGED 20-40). A QUESTIONNAIRE WAS PROVIDED TO THE SUBJECTS TO OBTAIN THEIR DEMOGRAPHIC AND OBSTETRICAL INFORMATION 6 WEEKS AFTER DELIVERY, AND THEIR HOSPITAL RECORDS WERE ALSO ASSESSED FOR FURTHER DETAILS. BASED ON THE NATURE AND DETAILS OBTAINED FOR THE ANTENATAL EXERCISES, SUBJECTS WERE DIVIDED INTO TWO GROUPS: CONTROL AND EXERCISE. OUTCOME MEASURES INCLUDED THE NEED FOR LABOR INDUCTION, SELF-PERCEIVED PAIN AND PERCEIVED EXERTION DURING LABOR, DURATION AND NATURE OF THE DELIVERY, NEWBORN INFANT WEIGHT, MATERNAL WEIGHT GAIN, HISTORY OF BACK PAIN, AND POST-PARTUM RECOVERY. THE TOTAL MATERNAL WEIGHT GAIN (IN KILOGRAMS) WAS CALCULATED FROM WEIGHT AT 6 WEEKS AFTER DELIVERY MINUS THE WEIGHT AT 12-14 WEEKS OF GESTATION. BACK PAIN DURING PREGNANCY AND SELF-PERCEIVED LABOR PAIN WERE MEASURED USING A VISUAL ANALOG SCALE (VAS). THE OVERALL PERCEIVED EXERTION DURING LABOR WAS MEASURED USING AN ADAPTED BORG SCALE FOR PERCEIVED EFFORT. RESULTS: THE SUBJECTS WHO FOLLOWED REGULAR ANTENATAL EXERCISES, INCLUDING YOGA, HAD SIGNIFICANTLY LOWER RATES OF CESAREAN SECTION, LOWER WEIGHT GAIN, HIGHER NEWBORN INFANT WEIGHT, LOWER PAIN AND OVERALL DISCOMFORT DURING LABOR, LOWER BACK PAIN THROUGHOUT PREGNANCY, AND EARLIER POST-PARTUM RECOVERY COMPARED TO THOSE WHO DID NO SPECIFIC EXERCISES OR ONLY WALKED DURING PREGNANCY. CONCLUSIONS: THIS RETROSPECTIVE STUDY SHOWED THAT REGULAR ANTENATAL EXERCISES, INCLUDING YOGA, RESULT IN BETTER OUTCOMES RELATED TO THE COURSE OF LABOR, DELIVERY, AND PREGNANCY. THESE RESULTS NOTABLY INDICATED THAT PREGNANT WOMEN SHOULD BE ACTIVE THROUGHOUT PREGNANCY AND FOLLOW A SUPERVISED EXERCISE PROGRAM THAT INCLUDES YOGA UNLESS CONTRAINDICATED. [PURPOSE] THE AIM OF THIS STUDY WAS TO INVESTIGATE THE EFFECTIVENESS OF YOGA IN THE MANAGEMENT OF CHRONIC NECK PAIN. [SUBJECTS AND METHODS] FIVE ELECTRONIC DATABASES WERE SEARCHED TO IDENTIFY RANDOMIZED CONTROLLED TRIALS (RCTS) OF YOGA INTERVENTION ON CHRONIC NECK PAIN. THE TRIALS WERE PUBLISHED IN THE ENGLISH LANGUAGE BETWEEN JANUARY 1966 AND DECEMBER 2015. THE COCHRANE RISK OF BIAS TOOL WAS USED TO ASSESS THE QUALITY OF THE TRIALS. [RESULTS] THREE TRIALS WERE IDENTIFIED AND INCLUDED IN THIS REVIEW. A CRITICAL APPRAISAL WAS PERFORMED ON THE TRIALS, AND THE RESULT INDICATED A HIGH RISK OF BIAS. A NARRATIVE DESCRIPTION WAS PROCESSED BECAUSE OF THE SMALL NUMBER OF RCTS. NECK PAIN INTENSITY AND FUNCTIONAL DISABILITY WERE SIGNIFICANTLY LOWER IN THE YOGA GROUPS THAN IN THE CONTROL GROUPS. [CONCLUSION] EVIDENCE FROM THE 3 RANDOMLY CONTROLLED TRIALS SHOWS THAT YOGA MAY BE BENEFICIAL FOR CHRONIC NECK PAIN. OBJECTIVE: TO INVESTIGATE THE EFFECTS OF AN ANTENATAL YOGA PROGRAM ON PERCEIVED MATERNAL LABOR PAIN AND DELIVERY OUTCOMES. MATERIAL & METHODS: THIS RANDOMIZED CONTROL TRIAL WAS CONDUCTED WITH SIXTY PRIMIPAROUS WOMEN, AGED 18-35 YEARS OLD, WHO WERE RANDOMLY ASSIGNED TO EITHER AN ANTENATAL YOGA PROGRAM OR CONTROL GROUPS. LABOR PAIN AND DISCOMFORT LEVEL OF THE PARTICIPANTS WERE MEASURED USING A VISUAL ANALOGUE SCALE AT CERVICAL DILATATION OF 3-4 C AND AT 2 AND 4 H AFTER THE INITIAL MEASUREMENT. DEMOGRAPHIC AND OBSTETRICAL INFORMATION WERE COLLECTED. THE ANTENATAL YOGA PROGRAM CONSISTED OF A 1-H SUPERVISED YOGA CLASS, THREE TIMES A WEEKLY, STARTING AT 26 WEEKS GESTATION. . RESULTS: PARTICIPANTS IN CONTROL GROUP REPORTED HIGHER PAIN INTENSITY COMPARED TO EXPERIMENTAL GROUP AT 3-4 CM OF DILATATION (P = 0.01) AND AT 2 H AFTER THE FIRST AND THE SECOND MEASUREMENTS (P = 0.000). MOTHERS IN THE ANTENATAL INTERVENTION GROUP THAT COMPLETED THE YOGA CLASS REQUIRED A DECREASED FREQUENCY OF LABOR INDUCTION IN COMPARISON WITH CONTROL GROUP (P = 0.008). IN ADDITION, MODE OF DELIVERY OF THE INTERVENTION GROUP RESULTED IN A LOWER PERCENTAGE OF CESAREAN SECTION THAN CONTROL GROUP (P = 0.002). LASTLY, THE INTERVENTION GROUP EXPERIENCED A SHORTER DURATION OF THE SECOND AND THIRD STAGES OF LABOR. INTERVAL LEVEL DATA WAS ANALYZED BY USING AN INDEPENDENT T-TEST AND CHI-SQUARE.
