1 140 173 A PROSPECTIVE STUDY ON TYPE-2 DIABETIC COMPLICATIONS AND EFFICACY OF INTEGRATED YOGA: A PAN INDIA 2017. BACKGROUND: TYPE 2 DIABETES (T2DM) CONTRIBUTES TO HIGH MORTALITY AND MORBIDITY BECAUSE OF ITS MAJOR COMPLICATIONS RELATED TO KIDNEY, HEART, BRAIN, AND EYES. IT ALSO POSES A HIGH RISK FOR MORTALITY BECAUSE OF COVID-19. STUDIES SUGGEST THE POSSIBLE IMPLICATIONS OF YOGA IN DELAYING OR ATTENUATING SUCH COMPLICATIONS. METHODOLOGY: THIS WAS A PAN-INDIA MULTI CENTERED CLUSTER-RANDOMIZED (4 LEVEL) TWO-ARMED TRIAL IN THE RURAL AND URBAN POPULATION OF ALL POPULOUS STATES OF INDIA. DATA WERE OBTAINED USING MOBILE APP IN ALL ADULTS IN THE HOUSEHOLD OF THE SELECTED CLUSTERS. RESULTS: WE REPORT THE DIABETES RELATED COMPLICATIONS IN 16623 ADULTS (48% MALES, 52% FEMALES) FROM 65 DISTRICTS (1 IN 10 DISTRICTS, 2011 CENSUS) OF 29 (OUT OF 35) STATES AND UNION TERRITORIES OF INDIA; MEAN AGE WAS 48.2 +/- 12.46 YEARS. OUT OF THIS 40% LIVED IN RURAL AND 62% IN URBAN LOCATIONS. IN HIGH RISK DIABETES INDIVIDUALS (SCORED >/= 60 POINTS ON INDIAN DIABETES RISK SCORE KEY), 18.0% HAD SELF-REPORTED HISTORY OF (PERIPHERAL NEUROPATHY, 6.1% HAD H/O MAJOR STROKES, 5.5% HAD MINOR STROKES (TRANSIENT ISCHEMIC EPISODES), 18.1% HAD LOWER LIMB CLAUDICATION, 20.5% LEG ULCERS, 4.4% HAD H/O CARDIAC SURGERY, 4.8% ANGIOPLASTY, AND 15.1% HAD DIABETES RETINOPATHY. COMPLICATIONS WERE HIGHER IN RURAL THAN IN URBAN AREAS, HIGHER IN PEOPLE WITH EXTENDED DURATION OF DIABETES. INTEGRATED YOGA MODULE FOR THREE MONTHS (ONE HOUR DAILY) SHOWED SIGNIFICANTLY BETTER REDUCTION IN SYMPTOMS RELATED TO COMPLICATIONS AS COMPARED TO CONTROL GROUP (P < .001). BACKGROUND: YOGA HAS POTENTIAL FOR BENEFIT FOR PATIENTS WITH CORONARY ARTERY DISEASE THOUGH OBJECTIVE, ANGIOGRAPHIC STUDIES ARE LACKING. MATERIAL AND METHODS: WE EVALUATED POSSIBLE ROLE OF LIFESTYLE MODIFICATION INCORPORATING YOGA, ON RETARDATION OF CORONARY ATHEROSCLEROTIC DISEASE. IN THIS PROSPECTIVE, RANDOMIZED, CONTROLLED TRIAL, 42 MEN WITH ANGIOGRAPHICALLY PROVEN CORONARY ARTERY DISEASE (CAD) WERE RANDOMIZED TO CONTROL (N = 21) AND YOGA INTERVENTION GROUP (N = 21) AND WERE FOLLOWED FOR ONE YEAR. THE ACTIVE GROUP WAS TREATED WITH A USER-FRIENDLY PROGRAM CONSISTING OF YOGA, CONTROL OF RISK FACTORS, DIET CONTROL AND MODERATE AEROBIC EXERCISE. THE CONTROL GROUP WAS MANAGED BY CONVENTIONAL METHODS I.E. RISK FACTOR CONTROL AND AMERICAN HEART ASSOCIATION STEP I DIET. RESULTS: AT ONE YEAR, THE YOGA GROUPS SHOWED SIGNIFICANT REDUCTION IN NUMBER OF ANGINAL EPISODES PER WEEK, IMPROVED EXERCISE CAPACITY AND DECREASE IN BODY WEIGHT. SERUM TOTAL CHOLESTEROL, LDL CHOLESTEROL AND TRIGLYCERIDE LEVELS ALSO SHOWED GREATER REDUCTIONS AS COMPARED WITH CONTROL GROUP. REVASCULARISATION PROCEDURES (CORONARY ANGIOPLASTY OR BYPASS SURGERY) WERE LESS FREQUENTLY REQUIRED IN THE YOGA GROUP (ONE VERSUS EIGHT PATIENTS; RELATIVE RISK = 5.45; P = 0.01). PURPOSE: THE PURPOSE OF THE PRESENT STUDY WAS TO ASSESS THE EFFECT OF HATHA YOGA TRAINING THAT WAS ADDED TO THE STANDARD CARDIAC REHABILITATION (CR) PROGRAM ON THE CARDIAC HEMODYNAMIC PARAMETERS AND PHYSICAL CAPACITY OF PATIENTS WITH ST-ELEVATION MYOCARDIAL INFARCTION (STEMI). METHODS: THE STUDY INCLUDED 70 MALE PATIENTS AGED 45-65 YR WITH STEMI WHO WERE TREATED BY ANGIOPLASTY. PATIENTS WERE RANDOMIZED TO STANDARD CR (CONTROL GROUP) VERSUS STANDARD CR PLUS HATHA YOGA (EXPERIMENTAL GROUP). THE STUDY GROUP PATIENTS WERE GIVEN A FAMILY BASED YOGA PROGRAMME WHICH INCLUDED, CONTROL OF RISK FACTORS, DIETARY MODIFICATIONS AND STRESS MANAGEMENT FOR A PERIOD OF ONE YEAR. AT THE END OF THE STUDY IMPROVEMENT IN ANXIETY SCORES WAS CONCORDANT WITH THE IMPROVEMENT SEEN IN THE MPI. NO UNTOWARD EFFECTS OF THE THERAPY WERE OBSERVED. THE COHORT WAS IDENTIFIED THROUGH THE INDIAN DIABETES RISK SCORING (IDRS) (>/= 60) AND A TOTAL OF 184 INDIVIDUALS WERE RANDOMIZED INTO INTERVENTION (N = 91) AND CONTROL GROUPS (N = 93). THE DYP GROUP UNDERWENT THE SPECIFIC DYP TRAINING WHEREAS THE CONTROL GROUP FOLLOWED THEIR DAILY REGIMEN. THE STUDY OUTCOMES INCLUDED CHANGES IN GLYCEMIC AND LIPID PROFILE. BACKGROUND: GIVEN THE SHORTAGE OF CARDIAC REHABILITATION (CR) PROGRAMS IN INDIA AND POOR UPTAKE WORLDWIDE, THERE IS AN URGENT NEED TO FIND ALTERNATIVE MODELS OF CR THAT ARE INEXPENSIVE AND MAY OFFER CHOICE TO SUBGROUPS WITH POOR UPTAKE (E.G., WOMEN AND ELDERLY). OBJECTIVES: THIS STUDY SOUGHT TO EVALUATE THE EFFECTS OF YOGA-BASED CR (YOGA-CARE) ON MAJOR CARDIOVASCULAR EVENTS AND SELF-RATED HEALTH IN A MULTICENTER RANDOMIZED CONTROLLED TRIAL. METHODS: THE TRIAL WAS CONDUCTED IN 24 MEDICAL CENTERS ACROSS INDIA. THIS STUDY RECRUITED 3,959 PATIENTS WITH ACUTE MYOCARDIAL INFARCTION WITH A MEDIAN AND MINIMUM FOLLOW-UP OF 22 AND 6 MONTHS. PATIENTS WERE INDIVIDUALLY RANDOMIZED TO RECEIVE EITHER A YOGA-CARE PROGRAM (N = 1,970) OR ENHANCED STANDARD CARE INVOLVING EDUCATIONAL ADVICE (N = 1,989). BACKGROUND: DYSLIPIDEMIA IS CONSIDERED A RISK FACTOR IN TYPE 2 DIABETES MELLITUS (T2DM) RESULTING IN CARDIO-VASCULAR COMPLICATIONS. YOGA PRACTICES HAVE SHOWN PROMISING RESULTS IN ALLEVIATING TYPE 2 DIABETES PATHOLOGY. METHOD: IN THIS STRATIFIED TRIAL ON A YOGA BASED LIFESTYLE PROGRAM IN CASES WITH TYPE 2 DIABETES, IN THE RURAL AND URBAN POPULATION FROM ALL ZONES OF INDIA, A TOTAL OF 17,012 ADULTS (>20 YEARS) OF BOTH GENDERS WERE SCREENED FOR LIPID PROFILE AND SUGAR LEVELS. THOSE WHO SATISFIED THE SELECTION CRITERIA WERE TAUGHT THE DIABETES YOGA PROTOCOL (DYP) FOR THREE MONTHS AND THE DATA WERE ANALYZED. RESULTS: AMONG THOSE WITH DIABETES, 29.1% HAD ELEVATED TOTAL CHOLESTEROL (TC > 200 MG/DL) LEVELS THAT WERE HIGHER IN URBAN (69%) THAN RURAL (31%) DIABETES PATIENTS. IT IS FURTHER ELEVATED WITH RISK FACTORS SUCH AS STRESS, ANXIETY, DEPRESSION, AN INCREASE IN TRIGLYCERIDES, DYSLIPIDEMIA, HYPERGLYCEMIA, HYPERTENSION, AND SO ON, WHICH MANIFESTS AS ATHEROSCLEROTIC DISEASE. YOGA-BASED LIFESTYLE INTERVENTION IS A NONINVASIVE EFFECTIVE TREATMENT METHOD TO CONTROL AND PREVENT CARDIAC RISK FACTORS IN CAD PATIENTS. YOGA HAS BEEN USED IN INDIA AS A THERAPEUTIC METHOD TO MANAGE HYPERTENSION AND OTHER CHRONIC DISORDERS AND IS FAST GAINING POPULARITY AS AN EFFECTIVE MEANS FOR THE ALLEVIATION OF STRESS, IMPROVEMENT OF FITNESS, AND ENHANCEMENT OF WELL-BEING. A PARALLEL, TWO ARM, RCT WAS CONDUCTED IN INTEGRAL HEALTH CLINIC (IHC), ALL INDIA INSTITUTE OF MEDICAL SCIENCES, INDIA FROM 2012 TO 2014. IHC IS AN OUTPATIENT FACILITY CONDUCTING YBLI PROGRAMS FOR PREVENTION AND MANAGEMENT OF CHRONIC DISEASES. TWO HUNDRED SIXTY MEN AND WOMEN (20-45 YEARS) VISITING THE OUTPATIENT DEPARTMENT OF A TERTIARY CARE HOSPITAL WERE DIAGNOSED WITH MET S AND RANDOMIZED 1:1 TO RECEIVE 12 WEEK YBLI (N = 130) OR DI (N = 130). PRIMARY OUTCOMES WERE CHANGE IN PLASMA LEVELS OF ADIPOKINES (LEPTIN, ADIPONECTIN, AND LEPTIN:ADIPONECTIN RATIO), MARKERS OF INFLAMMATION (TUMOR NECROSIS FACTOR [TNF]-ALPHA, INTERLEUKIN [IL]-6), MARKERS OF OXIDATIVE STRESS (THIOBARBITURIC ACID REACTIVE SUBSTANCES [TBARS], 8-HYDROXY-2'-DEOXYGUANOSINE [8-OHDG], AND SUPEROXIDE DISMUTASE [SOD]) MEASURED AT BASELINE, 2 WEEKS, AND 12 WEEKS. YBLI GROUP SHOWED A SIGNIFICANT DECREASE IN LEPTIN, LEPTIN:ADIPONECTIN RATIO, IL-6, 8-OHDG, AND TBARS LEVELS, WHEREAS THERE WAS A SIGNIFICANT INCREASE IN ADIPONECTIN AND SOD LEVELS. NO SIGNIFICANT CHANGES WERE NOTICED IN DI ALONE GROUP. YBLI SHOWED SIGNIFICANTLY GREATER REDUCTION IN TBARS LEVELS THAN IN DI GROUP, SUGGESTIVE OF REDUCED OXIDATIVE STRESS IN ADULTS WITH MET S. A 12 WEEK YBLI HAD A POSITIVE IMPACT ON OXIDATIVE STRESS VERSUS DI ALONE IN ADULTS WITH MET S. 2019 17 881 29 EFFECT OF YOGA TRAINING ON INFLAMMATORY CYTOKINES AND C-REACTIVE PROTEIN IN EMPLOYEES OF SMALL-SCALE INDUSTRIES. OBJECTIVE: THE PRESENT STUDY INTENDS TO SEE THE EFFECT OF YOGA PRACTICES ON LIPID PROFILE, INTERLEUKIN (IL)-6, TUMOR NECROSIS FACTOR (TNF)-ALPHA, AND HIGH-SENSITIVITY-C-REACTIVE PROTEIN (HS-CRP) AMONG APPARENTLY HEALTHY ADULTS EXPOSED TO OCCUPATIONAL HAZARDS. MATERIALS AND METHODS: IN THE PRESENT STUDY, 48 PARTICIPANTS AGED 30-58 YEARS (41.5 +/- 5.2) WHO WERE EXPOSED TO OCCUPATIONAL HAZARDS WERE RANDOMIZED INTO TWO GROUPS, THAT IS, EXPERIMENTAL AND WAIT-LIST CONTROL. ALL THE PARTICIPANTS WERE ASSESSED FOR LIPID PROFILE, IL-6, TNF-ALPHA, AND HS-CRP AT THE BASELINE AND AFTER COMPLETION OF 3 MONTHS OF YOGA TRAINING INTERVENTION. THE EXPERIMENTAL GROUP UNDERWENT YOGA TRAINING INTERVENTION FOR 1 H FOR 6 DAYS A WEEK FOR 3 MONTHS, WHEREAS CONTROL GROUP CONTINUED WITH THEIR DAILY ACTIVITIES EXCEPT YOGA TRAINING. DATA ANALYSIS WAS DONE USING STATISTICAL SOFTWARE SPSS VERSION 20.0. DATA WERE ANALYZED USING PAIRED T-TESTS AND INDEPENDENT T-TEST. RESULTS: THE RESULTS OF WITHIN GROUP COMPARISON REVEALED HIGHLY SIGNIFICANT CHANGES IN CHOLESTEROL (P < 0.001), HIGH-DENSITY LIPOPROTEIN (P < 0.001), LOW-DENSITY LIPOPROTEIN (LDL)(P < 0.01), HS-CRP (P < 0.01), IL-6 (P < 0.001), AND TNF-ALPHA (P < 0.001) IN EXPERIMENTAL GROUP. COMPARISON BETWEEN EXPERIMENTAL AND CONTROL GROUP REVEALED SIGNIFICANT CHANGES IN CHOLESTEROL (P < 0.01), LDL (P < 0.05), IL-6 (P < 0.01), TNF-ALPHA (P < 0.01), AND HS-CRP (P < 0.01). CONCLUSION: A YOGA-BASED LIFESTYLE INTERVENTION SEEMS TO BE A HIGHLY PROMISING ALTERNATIVE THERAPY WHICH FAVORABLY ALTERS INFLAMMATORY MARKERS AND METABOLIC RISK FACTORS. 2017 18 50 34 A COMPARATIVE CONTROLLED TRIAL COMPARING THE EFFECTS OF YOGA AND WALKING FOR OVERWEIGHT AND OBESE ADULTS. BACKGROUND: WALKING AND YOGA HAVE BEEN INDEPENDENTLY EVALUATED FOR WEIGHT CONTROL; HOWEVER, THERE ARE VERY FEW STUDIES COMPARING THE 2 WITH RANDOMIZATION. MATERIAL AND METHODS: THE PRESENT STUDY COMPARED THE EFFECTS OF 90 MINUTES/DAY FOR 15 DAYS OF SUPERVISED YOGA OR SUPERVISED WALKING ON: (I) RELATED BIOCHEMISTRY, (II) ANTHROPOMETRIC VARIABLES, (III) BODY COMPOSITION, (IV) POSTURAL STABILITY, AND (V) BILATERAL HAND GRIP STRENGTH IN OVERWEIGHT AND OBESE PERSONS. SIXTY-EIGHT PARTICIPANTS, OF WHOM 5 WERE OVERWEIGHT (BMI >/=25 KG/M2) AND 63 WERE OBESE (BMI >/=30 KG/M2; GROUP MEAN AGE +/-S.D., 36.4+/-11.2 YEARS; 35 FEMALES), WERE RANDOMIZED AS 2 GROUPS - (I) A YOGA GROUP AND (II) A WALKING GROUP - GIVEN THE SAME DIET. RESULTS: ALL DIFFERENCES WERE PRE-POST CHANGES WITHIN EACH GROUP. BOTH GROUPS SHOWED A SIGNIFICANT (P<0.05; REPEATED MEASURES ANOVA, POST-HOC ANALYSES) DECREASE IN: BMI, WAIST CIRCUMFERENCE, HIP CIRCUMFERENCE, LEAN MASS, BODY WATER, AND TOTAL CHOLESTEROL. OBJECTIVE: TO DETERMINE THE EFFICACY OF A 12-WEEK HATHA YOGA INTERVENTION TO IMPROVE METABOLIC RISK PROFILES AND HEALTH-RELATED QUALITY OF LIFE (HRQOL) IN CHINESE ADULTS WITH AND WITHOUT METABOLIC SYNDROME (METS). METHODS: WE CONDUCTED A CONTROLLED TRIAL WITHIN AN UNIVERSITY-AFFILIATED HOSPITAL. 173 CHINESE MEN AND WOMEN AGED 18 OR ABOVE WERE ASSIGNED TO EITHER THE YOGA INTERVENTION GROUP (N = 87) OR THE CONTROL GROUP (N = 86). PRIMARY OUTCOMES INCLUDED 12-WEEK CHANGE IN METABOLIC RISK FACTORS AND METS Z SCORE. SECONDARY OUTCOME WAS HRQOL (MEDICAL OUTCOMES SHORT FORM SURVEY AT 12 WEEKS). RESULTS: THE MEAN AGE OF PARTICIPANTS WAS 52.0 (SD 7.4, RANGE 31-71) YEARS. ANALYSIS INVOLVING THE ENTIRE STUDY POPULATION REVEALED THAT THE YOGA GROUP ACHIEVED GREATER DECLINE IN WAIST CIRCUMFERENCE (P<0.001), FASTING GLUCOSE (P<0.01), TRIGLYCERIDES (P<0.05), AND METS Z SCORE (P<0.01). YOGA TRAINING ALSO IMPROVED GENERAL HEALTH PERCEPTIONS (P<0.01), PHYSICAL COMPONENT SCORE (P<0.01), AND SOCIAL FUNCTIONING (P<0.01) DOMAINS SCORE OF HRQOL. HOWEVER, NO SIGNIFICANT DIFFERENCES BETWEEN GROUPS WERE OBSERVED IN THE MEAN CHANGE OF SYSTOLIC/DIASTOLIC BLOOD PRESSURES OR HIGH-DENSITY LIPID PROTEIN CHOLESTEROL (ALL P>0.05). INTRODUCTION: INDIA HAS BECOME THE EPICENTRE FOR DIABETES, A STRESS-RELATED DISORDER AFFECTING THE WORKING SKILLS AND DAY-TO-DAY LIFESTYLE MANAGEMENT OF YOUNGER POPULATION. MOST OF THE STUDIES HAVE REPORTED THE EFFECT OF YOGA ON IMPROVING QUALITY OF LIFE (QOL) IN DIABETIC PATIENTS WITH OTHER COMORBIDITIES. TILL DATE, NO RANDOMIZED CONTROL TRIAL REPORTS ARE AVAILABLE TO SHOW THE EFFECT OF YOGA THERAPY ON QOL AND INDIAN DIABETES RISK SCORE (IDRS) IN NORMOTENSIVE PREDIABETIC AND DIABETIC YOUNG INDIVIDUALS. AIM: TO DETERMINE THE EFFECT OF 12 WEEKS OF YOGA THERAPY ON QOL AND IDRS AMONG NORMOTENSIVE PREDIABETIC AND DIABETIC YOUNG INDIAN ADULTS. MATERIALS AND METHODS: A RANDOMIZED CONTROL TRIAL WAS CONDUCTED IN ENDOCRINOLOGY OUTPATIENT DEPARTMENT (OPD). NORMOTENSIVE PARTICIPANTS (N=310) AGED 18-45 YEARS WERE DIVIDED INTO HEALTHY CONTROLS (N=62), PREDIABETICS (N=124) AND DIABETICS (N=124). STUDY GROUP SUBJECTS WERE RANDOMLY ASSIGNED TO GROUP II (N=62, PREDIABETES-STANDARD TREATMENT), GROUP III (N=62, PREDIABETES-STANDARD TREATMENT + YOGA THERAPY), GROUP IV (N=62, DIABETES-STANDARD TREATMENT) AND GROUP V (N=62, DIABETES-STANDARD TREATMENT + YOGA THERAPY). FLANAGAN QOL SCALE, IDRS QUESTIONNAIRE, FASTING PLASMA GLUCOSE (FPG) AND INSULIN WERE ASSESSED PRE AND POST 12 WEEKS OF INTERVENTION. STATISTICAL ANALYSIS WAS DONE USING STUDENT'S PAIRED T-TEST AND ONE-WAY ANOVA. RESULTS: PRE-POST INTERVENTION ANALYSIS SHOWED SIGNIFICANT IMPROVEMENT IN QOL SCALE WITH P<0.01 IN GROUP II AND GROUP IV; P<0.001 IN GROUP III AND GROUP V RESPECTIVELY. THERE WAS SIGNIFICANT REDUCTION IN IDRS IN GROUP II (P<0.05); P<0.001 IN GROUP III, GROUP IV AND GROUP V RESPECTIVELY. SIGNIFICANT DIFFERENCE (P<0.001) IN QOL SCALE AND IDRS WERE FOUND WHEN STUDY GROUPS WITH STANDARD TREATMENT ALONG WITH YOGA THERAPY WERE COMPARED TO STANDARD TREATMENT ALONE. CONCLUSION: YOGA THERAPY ALONG WITH STANDARD TREATMENT FOR 12 WEEKS IMPROVED QOL AND ATTENUATED THE DIABETES RISK AMONG INDIAN PREDIABETICS AND DIABETICS COMPARED TO STANDARD TREATMENT ALONE. 2017
