OUR OBJECTIVE WAS TO EXAMINE THE FEASIBILITY AND SAFETY OF CONDUCTING A RANDOMIZED CONTROLLED TRIAL OF YOGA FOR OLDER ADULTS AT RISK FOR MOBILITY LIMITATIONS. METHODS: SEDENTARY OLDER ADULTS (N = 46; AGE 60-89) WERE RECRUITED AND RANDOMIZED TO EITHER YOGA OR A HEALTH EDUCATION COMPARISON GROUP. YOGA SESSIONS (60-MIN) OCCURRED 2X WEEKLY, AND 90-MIN HEALTH EDUCATION SESSIONS OCCURRED WEEKLY, FOR 10 WEEKS. THE PRIMARY OUTCOMES WERE RECRUITMENT RATE, INTERVENTION ATTENDANCE, AND RETENTION AT ASSESSMENTS. ADVERSE EVENT RATES AND PARTICIPANT SATISFACTION WERE ALSO MEASURED. PHYSICAL PERFORMANCE MEASURES OF GAIT, BALANCE, AND STRENGTH AND SELF-REPORT OUTCOME MEASURES WERE ADMINISTERED AT BASELINE AND 10-WEEKS. RESULTS: RECRUITMENT LASTED 6 MONTHS. RETENTION OF PARTICIPANTS AT THE 10-WEEK FOLLOW-UP WAS HIGH (89% - PERFORMANCE MEASURES; 98% - SELF-REPORT QUESTIONNAIRES). ATTENDANCE WAS GOOD WITH 82% OF YOGA AND 74% OF HEALTH EDUCATION PARTICIPANTS ATTENDING AT LEAST 50% OF THE SESSIONS. NO SERIOUS ADVERSE EVENTS WERE REPORTED. PATIENT SATISFACTION WITH THE INTERVENTIONS WAS HIGH. THE MEAN EFFECT SIZE FOR THE PHYSICAL PERFORMANCE MEASURES WAS 0.35 WITH SOME OVER 0.50. THE MEAN EFFECT SIZE FOR SELF-REPORT OUTCOME MEASURES WAS 0.36. TWICE-WEEKLY 75-MINUTE HATHA YOGA CLASSES FOR 95 PREDOMINANTLY LOW-INCOME MINORITY ADULTS WITH NONSPECIFIC CLBP. PARTICIPANT CHARACTERISTICS COLLECTED AT BASELINE WERE USED TO DETERMINE FACTORS BEYOND TREATMENT ASSIGNMENT (REPORTED IN THE INITIAL STUDY) THAT PREDICTED OUTCOME. WE USED BIVARIATE TESTING TO IDENTIFY BASELINE CHARACTERISTICS ASSOCIATED WITH IMPROVEMENT IN FUNCTION AND PAIN, AND INCLUDED SELECT FACTORS IN A MULTIVARIATE LINEAR REGRESSION. GREATER THAN HIGH SCHOOL EDUCATION LEVEL, CLBP LESS THAN 1 YEAR, AND LOWER BASELINE SF36 PCS WERE INDEPENDENTLY ASSOCIATED WITH IMPROVEMENT IN PAIN INTENSITY. OTHER DEMOGRAPHICS INCLUDING RACE, INCOME, GENDER, BMI, AND USE OF PAIN MEDICATIONS WERE NOT ASSOCIATED WITH EITHER OUTCOME. CONCLUSIONS: POOR PHYSICAL HEALTH AT BASELINE IS ASSOCIATED WITH GREATER IMPROVEMENT FROM YOGA IN BACK-RELATED FUNCTION AND PAIN. TWO PATIENTS WITHDREW, ONE DUE TO A FALL OUTSIDE THE INSTITUTION AND THE OTHER DUE TO LACK OF TIME AND INTEREST. THERE WERE NO ADVERSE EVENTS DURING THE YOGA SESSIONS. RESULTS: A TOTAL OF 27 RECORDS FROM 22 RCTS WERE INCLUDED (17 RCTS ASSESSED PHYSICAL FUNCTION AND 20 ASSESSED HRQOL). THE META-ANALYSIS REVEALED SIGNIFICANT EFFECTS (5% LEVEL OF SIGNIFICANCE) FAVOURING THE YOGA GROUP FOR THE FOLLOWING PHYSICAL FUNCTION OUTCOMES COMPARED WITH INACTIVE CONTROLS: BALANCE (EFFECT SIZE (ES) = 0.7), LOWER BODY FLEXIBILITY (ES = 0.5), LOWER LIMB STRENGTH (ES = 0.45); COMPARED WITH ACTIVE CONTROLS: LOWER LIMB STRENGTH (ES = 0.49), LOWER BODY FLEXIBILITY (ES = 0.28). FOR HRQOL, SIGNIFICANT EFFECTS FAVOURING YOGA WERE FOUND COMPARED TO INACTIVE CONTROLS FOR: DEPRESSION (ES = 0.64), PERCEIVED MENTAL HEALTH (ES = 0.6), PERCEIVED PHYSICAL HEALTH (ES = 0.61), SLEEP QUALITY (ES = 0.65), AND VITALITY (ES = 0.31); COMPARED TO ACTIVE CONTROLS: DEPRESSION (ES = 0.54). CONCLUSION: THIS REVIEW IS THE FIRST TO COMPARE THE EFFECTS OF YOGA WITH ACTIVE AND INACTIVE CONTROLS IN OLDER ADULTS NOT CHARACTERISED BY A SPECIFIC CLINICAL CONDITION. RESULTS INDICATE THAT YOGA INTERVENTIONS IMPROVE MULTIPLE PHYSICAL FUNCTION AND HRQOL OUTCOMES IN THIS POPULATION COMPARED TO BOTH CONTROL CONDITIONS. TRIAL REGISTRATION: PROSPERO REGISTRATION NUMBER: CRD42016038052 . 2019 18 461 37 CHANGES IN PERCEIVED STRESS AFTER YOGA, PHYSICAL THERAPY, AND EDUCATION INTERVENTIONS FOR CHRONIC LOW BACK PAIN: A SECONDARY ANALYSIS OF A RANDOMIZED CONTROLLED TRIAL. OBJECTIVE: PERCEIVED STRESS AND MUSCULOSKELETAL PAIN ARE COMMON, ESPECIALLY IN LOW-INCOME POPULATIONS. STUDIES EVALUATING TREATMENTS TO REDUCE STRESS IN PATIENTS WITH CHRONIC PAIN ARE LACKING. WE AIMED TO QUANTIFY THE EFFECT OF TWO EVIDENCE-BASED INTERVENTIONS FOR CHRONIC LOW BACK PAIN (CLBP), YOGA AND PHYSICAL THERAPY (PT), ON PERCEIVED STRESS IN ADULTS WITH CLBP. METHODS: WE USED DATA FROM AN ASSESSOR-BLINDED, PARALLEL-GROUP RANDOMIZED CONTROLLED TRIAL, WHICH RECRUITED PREDOMINANTLY LOW-INCOME AND RACIALLY DIVERSE ADULTS WITH CLBP. PARTICIPANTS (N = 320) WERE RANDOMLY ASSIGNED TO 12 WEEKS OF YOGA, PT, OR BACK PAIN EDUCATION. WE COMPARED CHANGES IN THE 10-ITEM PERCEIVED STRESS SCALE (PSS-10) FROM BASELINE TO 12- AND 52-WEEK FOLLOW-UP AMONG YOGA AND PT PARTICIPANTS WITH THOSE RECEIVING EDUCATION. SUBANALYSES WERE CONDUCTED FOR PARTICIPANTS WITH ELEVATED PRE-INTERVENTION PERCEIVED STRESS (PSS-10 SCORE >/=17). WE CONDUCTED SENSITIVITY ANALYSES USING VARIOUS IMPUTATION METHODS TO ACCOUNT FOR POTENTIAL BIASES IN OUR ESTIMATES DUE TO MISSING DATA. RESULTS: AMONG 248 PARTICIPANTS (MEAN AGE = 46.4 YEARS, 80% NONWHITE) COMPLETING ALL THREE SURVEYS, YOGA AND PT SHOWED GREATER REDUCTIONS IN PSS-10 SCORES COMPARED WITH EDUCATION AT 12 WEEKS (MEAN BETWEEN-GROUP DIFFERENCE = -2.6, 95% CONFIDENCE INTERVAL [CI] = -4.5 TO -0.66, AND MEAN BETWEEN-GROUP DIFFERENCE = -2.4, 95% CI = -4.4 TO -0.48, RESPECTIVELY). THIS EFFECT WAS STRONGER AMONG PARTICIPANTS WITH ELEVATED PRE-INTERVENTION PERCEIVED STRESS. BETWEEN-GROUP EFFECTS HAD ATTENUATED BY 52 WEEKS. RESULTS WERE SIMILAR IN SENSITIVITY ANALYSES. CONCLUSIONS: YOGA AND PT WERE MORE EFFECTIVE THAN BACK PAIN EDUCATION FOR REDUCING PERCEIVED STRESS AMONG LOW-INCOME ADULTS WITH CLBP. 2020 19 1781 44 PREDICTORS OF AND BARRIERS TO ADHERENCE IN A 16-WEEK RANDOMISED CONTROLLED TRIAL OF BIKRAM YOGA IN STRESSED AND SEDENTARY ADULTS. BACKGROUND: BIKRAM YOGA MAY ENHANCE HEALTH OUTCOMES IN HEALTHY ADULTS AND THOSE AT RISK FOR CHRONIC DISEASE, HOWEVER, CHALLENGES REMAIN IN ACHIEVING OPTIMAL ADHERENCE TO THIS PRACTICE. THIS STUDY INVESTIGATED FACTORS INFLUENCING ADHERENCE TO A 16-WEEK BIKRAM YOGA INTERVENTION IN STRESSED AND SEDENTARY ADULTS. METHODS: EXPERIMENTAL GROUP PARTICIPANTS (N = 29) WERE INSTRUCTED TO ATTEND 3-5 BIKRAM YOGA CLASSES WEEKLY FOR 16 WEEKS. BASELINE DEMOGRAPHICS, BEHAVIOURS AND HEALTH MEASURES WERE INVESTIGATED AS PREDICTORS OF ADHERENCE. BARRIERS WERE ASSESSED VIA DOCUMENTATION OF ADVERSE EVENTS, AND EXIT SURVEY RESPONSES. RESULTS: PARTICIPANTS (38.2 +/- 10.1 YEARS) WERE PREDOMINANTLY OVERWEIGHT-OBESE (83%), FEMALE (79%), AND ATTENDED 27 +/- 18 CLASSES. HIGHER ADHERENCE WAS ASSOCIATED WITH OLDER AGE (P = 0.094), LESS PAIN (P = 0.011), FEWER PHYSICAL LIMITATIONS (P = 0.011), POORER BLOOD LIPID PROFILE, AND HIGHER HEART RATE VARIABILITY (HRV; TOTAL POWER, (P = 0.097)). IN MULTI-VARIABLE ANALYSIS, THREE VARIABLES: AGE (BETA = 0.492, P = 0.006), HRV (BETA = 0.413, P = 0.021) AND PAIN (BETA = 0.329, P = 0.048) REMAINED PREDICTORS OF ADHERENCE. DIFFICULTY COMMITTING TO THE TRIAL, LACK OF ENJOYMENT AND ADVERSE EVENTS WERE BARRIERS TO ADHERENCE. CONCLUSIONS: THESE FINDINGS SHOULD BE CONSIDERED IN THE DEVELOPMENT OF FUTURE BIKRAM YOGA TRIALS TO FACILITATE HIGHER LEVELS OF ADHERENCE, WHICH MAY ENHANCE HEALTH OUTCOMES AND INFORM COMMUNITY PRACTICE. CONTEXT: YOGA IMPROVES QUALITY OF LIFE IN ELDERS >/=65 YEARS, BUT STUDIES AMONG ELDERS WITH CHRONIC PAIN ARE LIMITED. OBJECTIVE: CONDUCT A FEASIBILITY STUDY OF GENTLE YOGA AMONG ELDERS IN ASSISTED AND INDEPENDENT LIVING. DESIGN: SINGLE ARM PRE/POST CLINICAL TRIAL. SUBJECTS: ADULTS (>/=65 YEARS OF AGE) WITH SELF-IDENTIFIED CHRONIC PAIN (>/=3 ON A 10-POINT SCALE, LASTING FOR >/=3 MONTHS) AND NO CURRENT YOGA PRACTICE. INTERVENTION: TEN WEEKLY 60-MIN GENTLE YOGA CLASSES TAILORED TO ELDERLY ADULTS. OUTCOME MEASURES: AT BASELINE, WEEKS 5, 10 (END OF INTERVENTION), AND 20 (FOLLOW-UP), WE COLLECTED DATA ON FEASIBILITY (ADHERENCE, RETENTION, SAFETY), PAIN, ANXIETY, DEPRESSION, FATIGUE, SLEEP DISTURBANCE, AND PHYSICAL FUNCTION. RESULTS: TWENTY-SIX PARTICIPANTS ENROLLED (88% WOMEN, 77% WHITE, 58% IN ASSISTED LIVING) WITH AVERAGE AGE OF 86.6 +/- 4.4 (MEAN, STD). TWENTY PARTICIPANTS COMPLETED THE INTERVENTION, WITH 90% ADHERING (COMPLETING >/=6 CLASSES). NINE PARTICIPANTS (45% OF COMPLETERS) EXPERIENCED ADVERSE EVENTS, WHICH WERE NON-SERIOUS AND RELATED TO TRANSIENT MUSCULOSKELETAL PAIN. NO ADVERSE EVENTS RESULTED IN STUDY WITHDRAWAL. PARTICIPANTS REPORTED BEING SOMEWHAT LIKELY TO RECOMMEND YOGA TO A FRIEND, AND QUITE A BIT LIKELY TO DO YOGA AGAIN. AT THE END OF THE INTERVENTION, FOUR OF TWENTY PARTICIPANTS REPORTED PRACTICING YOGA OUTSIDE OF CLASS. ANXIETY SIGNIFICANTLY DECREASED FROM 5.80 (SE=0.90) TO 4.44 (SE=0.74) (P = 0.014), BUT THERE WERE NO CHANGES IN OTHER MEASURES. CONCLUSIONS: OUR PILOT 10-WEEK YOGA STUDY WAS GENERALLY SAFE FOR AND SUITABLE TO ASSISTED AND INDEPENDENT LIVING ELDERLY ADULTS. FUTURE STUDIES ARE NEEDED TO EXAMINE OTHER EFFECTS OF YOGA IN ASSISTED/INDEPENDENT LIVING ADULTS WITH CHRONIC PAIN. 2022
