1 90 135 A MODIFIED YOGA-BASED EXERCISE PROGRAM IN HEMODIALYSIS PATIENTS: A RANDOMIZED CONTROLLED STUDY. AIM: TO EVALUATE THE EFFECTS OF A YOGA-BASED EXERCISE PROGRAM ON PAIN, FATIGUE, SLEEP DISTURBANCE, AND BIOCHEMICAL MARKERS IN HEMODIALYSIS PATIENTS. MATERIALS AND METHODS: IN 2004 A RANDOMIZED CONTROLLED TRIAL WAS CARRIED OUT IN THE OUTPATIENT HEMODIALYSIS UNIT OF THE NEPHROLOGY DEPARTMENT, ULUDAG UNIVERSITY FACULTY OF MEDICINE. CLINICALLY STABLE HEMODIALYSIS PATIENTS (N=37) WERE INCLUDED AND FOLLOWED IN TWO GROUPS: THE MODIFIED YOGA-BASED EXERCISE GROUP (N=19) AND THE CONTROL GROUP (N=18). YOGA-BASED EXERCISES WERE DONE IN GROUPS FOR 30 MIN/DAY TWICE A WEEK FOR 3 MONTHS. ALL OF THE PATIENTS IN THE YOGA AND CONTROL GROUPS WERE GIVEN AN ACTIVE RANGE OF MOTION EXERCISES TO DO FOR 10 MIN AT HOME. THE MAIN OUTCOME MEASURES WERE PAIN INTENSITY (MEASURED BY THE VISUAL ANALOGUE SCALE, VAS), FATIGUE (VAS), SLEEP DISTURBANCE (VAS), AND GRIP STRENGTH (MMHG); BIOCHEMICAL VARIABLES-- UREA, CREATININE, CALCIUM, ALKALINE PHOSPHATASE, PHOSPHORUS, CHOLESTEROL, HDL-CHOLESTEROL, TRIGLYCERIDE, ERYTHROCYTE, HEMATOCRIT--WERE EVALUATED. RESULTS: AFTER A 12-WEEK INTERVENTION, SIGNIFICANT IMPROVEMENTS WERE SEEN IN THE VARIABLES: PAIN -37%, FATIGUE -55%, SLEEP DISTURBANCE -25%, GRIP STRENGTH +15%, UREA -29%, CREATININE -14%, ALKALINE PHOSPHATASE -15%, CHOLESTEROL -15%, ERYTHROCYTE +11%, AND HEMATOCRIT COUNT +13%; NO SIDE-EFFECTS WERE SEEN. IMPROVEMENT OF THE VARIABLES IN THE YOGA-BASED EXERCISE PROGRAM WAS FOUND TO BE SUPERIOR TO THAT IN THE CONTROL GROUP FOR ALL THE VARIABLES EXCEPT CALCIUM, PHOSPHORUS, HDL-CHOLESTEROL AND TRIGLYCERIDE LEVELS. REGULAR MONITORING OF BLOOD PRESSURE, RENAL FUNCTION, REQUIREMENT OF A NUMBER OF DIALYSIS, AND QUALITY OF LIFE (QOL) INDICATORS WERE DONE. FIFTY PATIENTS (YOGA - 25; CONTROL-25) COMPLETED 6 MONTHS FOLLOW-UP. RESULTS: IN YOGA GROUP, A SIGNIFICANT REDUCTION OF SYSTOLIC AND DIASTOLIC BLOOD PRESSURE, SIGNIFICANT REDUCTION IN BLOOD UREA AND SERUM CREATININE LEVELS, AND SIGNIFICANT IMPROVEMENT IN PHYSICAL AND PSYCHOLOGICAL DOMAIN OF THE WORLD HEALTH ORGANIZATION QOL (AS ASSESSED BY BREF QOL SCORES) WERE SEEN AFTER 6 MONTHS. IN CONTROL GROUP, RISE OF BLOOD PRESSURE, DETERIORATION OF RENAL FUNCTION, AND QOL WERE OBSERVED. POSTSTUDY COMPARISON BETWEEN THE TWO GROUPS SHOWED A STATISTICALLY SIGNIFICANT REDUCTION OF BLOOD PRESSURE, NONSIGNIFICANT REDUCTION IN BLOOD UREA AND SERUM CREATININE, AND SIGNIFICANT IMPROVEMENT IN PHYSICAL AND PSYCHOLOGICAL DOMAIN OF QOL IN YOGA GROUP AS COMPARED TO CONTROL GROUP. RESULTS: FOLLOWING THE 10 WEEK INTERVENTION STRESS, ANXIETY AND QUALITY OF LIFE SCORES IMPROVED OVER TIME. BACKGROUND: YOGA SEEMS TO EXERT ITS EFFECT AGAINST ARTERIAL HYPERTENSION MAINLY THROUGH THE ASSOCIATED BREATHING AND MEDITATION TECHNIQUES, AND LESS SO THROUGH YOGA POSTURES. THE GOAL OF THIS TRIAL WAS TO COMPARE THE BLOOD PRESSURE-LOWERING EFFECT OF YOGA INTERVENTIONS WITH AND WITHOUT YOGA POSTURES IN PATIENTS WITH ARTERIAL HYPERTENSION. METHODS: 75 PATIENTS TAKING MEDICATIONS FOR ARTERIAL HYPERTENSION (72% WOMEN, MEAN AGE 58.7 +/- 9.5 YEARS) WERE RANDOMIZED INTO THREE GROUPS: A YOGA INTERVENTION GROUP WITH YOGA POSTURES (25 PATIENTS, OF WHOM 5 DROPPED OUT OF THE TRIAL BEFORE ITS END), A YOGA INTERVENTION GROUP WITHOUT YOGA POSTURES (25 PATIENTS, 3 DROPOUTS), AND A WAIT LIST CONTROL GROUP (25 PATIENTS, ONE DROPOUT). THE INTERVENTIONS CONSISTED OF 90 MINUTES OF YOGA PRACTICE PER WEEK FOR TWELVE WEEKS. THE DATA COLLECTORS, WHO WERE BLINDED TO THE INTERVENTION RECEIVED, ASSESSED THE PRIMARY OUTCOME MEASURES "SYSTOLIC 24-HOUR BLOOD PRESSURE" AND "DIASTOLIC 24-HOUR BLOOD PRESSURE" BEFORE AND AFTER THE INTERVENTION. IN THIS REPORT, WE ALSO PRESENT THE FINDINGS ON SECONDARY OUTCOME MEASURES, INCLUDING FOLLOW-UP DATA. RESULTS: AFTER THE INTERVENTION, THE SYSTOLIC 24-HOUR BLOOD PRESSURE IN THE YOGA INTERVENTION GROUP WITHOUT YOGA POSTURES WAS SIGNIFICANTLY LOWER THAN IN THE CONTROL GROUP (GROUP DIFFERENCE [DELTA]= -3.8 MMHG; [95% CONFIDENCE INTERVAL (CI): (-0.3; -7.4) P = 0.035]); IT WAS ALSO SIGNIFICANTLY LOWER THAN IN THE YOGA INTERVENTION GROUP WITH YOGA POSTURES (DELTA = -3.2 MMHG; 95% CI: [-6.3; -0.8]; P = 0.045). DIASTOLIC BLOOD PRESSURES DID NOT DIFFER SIGNIFICANTLY ACROSS GROUPS. NO SERIOUS ADVERSE EVENTS WERE ENCOUNTERED IN THE COURSE OF THE TRIAL. CONCLUSION: IN ACCORDANCE WITH THE FINDINGS OF EARLIER STUDIES, WE FOUND THAT ONLY YOGA WITHOUT YOGA POSTURES INDUCED A SHORT-TERM LOWERING OF AMBULATORY SYSTOLIC BLOOD PRESSURE. AIM: THE AIM OF THE STUDY WAS TO COMPARE THE EFFECTS OF AEROBIC EXERCISE TRAINING AND YOGA ON THE FUNCTIONAL CAPACITY, PERIPHERAL MUSCLE STRENGTH, QUALITY OF LIFE (QOL), AND FATIGUE IN BREAST CANCER SURVIVORS. MATERIAL AND METHODS: A TOTAL OF 52 PATIENTS WITH A DIAGNOSIS OF BREAST CANCER WERE INCLUDED IN THE STUDY. THE PATIENTS WERE RANDOMLY ASSIGNED TO 2 GROUPS: AEROBIC EXERCISE (N = 28) AND YOGA ADDED TO AEROBIC EXERCISE (N = 24). BOTH GROUPS PARTICIPATED IN SUBMAXIMAL EXERCISE 30 MINUTES/D, 3 D/WK FOR 6 WEEKS. THE SECOND GROUP PARTICIPATED IN A 1-HOUR YOGA PROGRAM IN ADDITION TO AEROBIC EXERCISE TRAINING. FUNCTIONAL CAPACITY WAS ASSESSED BY THE 6-MINUTE WALK TEST (6MWT). PERIPHERAL MUSCLE STRENGTH WAS EVALUATED WITH A HAND-HELD DYNAMOMETER. THE FATIGUE SEVERITY LEVEL WAS ASSESSED WITH THE FATIGUE SEVERITY SCALE (FSS). THE QOL WAS DETERMINED BY THE EUROPEAN ORGANISATION FOR RESEARCH AND TREATMENT OF CANCER QUALITY OF LIFE QUESTIONNAIRE. RESULTS: THERE WERE STATISTICALLY SIGNIFICANT INCREASES IN PERIPHERAL MUSCLE STRENGTH, THE 6MWT DISTANCE, AND THE PERCEPTION OF QOL IN BOTH GROUPS (P < .05). ADDITIONALLY, THE GROUP WITH AEROBIC EXERCISE AND YOGA SHOWED MARKED IMPROVEMENT COMPARED WITH THE AEROBIC EXERCISE GROUP IN FATIGUE PERCEPTION (P < .05). CONCLUSION: ACCORDING TO THE DATA FROM THIS STUDY, AEROBIC EXERCISE TRAINING AND YOGA IMPROVED THE FUNCTIONAL CAPACITY AND QOL OF BREAST CANCER PATIENTS. OBJECTIVES: TO OBSERVE THE EFFECT OF YOGA ON MENOPAUSAL SYMPTOMS USING A PROSPECTIVE, RANDOMIZED, CONTROLLED AND INTERVENTIONAL STUDY. MAIN OUTCOME MEASURES TOTAL MENOPAUSE RATING SCALE (MRS) SCORE AND THREE SUBSCALE SCORES (SOMATOVEGETATIVE, PSYCHOLOGICAL AND UROGENITAL) WERE MEASURED ON DAY 1 AND DAY 90 IN THE STUDY GROUP WHICH PERFORMED YOGA (ASANA, PRANAYAM AND MEDITATION) UNDER SUPERVISION FOR THREE MONTHS, AND WERE COMPARED WITH THE CONTROL GROUP THAT DID NOT PERFORM YOGA. MRS HAS BEEN DESIGNED TO MEASURE HEALTH-RELATED QUALITY OF LIFE OF AGEING WOMEN. IT CONSISTS OF 11 SYMPTOMS AND THREE SUBSCALES. RESULTS: IT WAS OBSERVED THAT ON DAY 1 THE SCORES IN BOTH THE GROUPS WERE COMPARABLE. BACKGROUND: RESEARCH SUGGESTS THAT STRESS-REDUCTION PROGRAMS TAILORED TO THE CANCER SETTING HELP PATIENTS COPE WITH THE EFFECTS OF TREATMENT AND IMPROVE THEIR QUALITY OF LIFE. YOGA, AN ANCIENT EASTERN SCIENCE, INCORPORATES STRESS-REDUCTION TECHNIQUES THAT INCLUDE REGULATED BREATHING, VISUAL IMAGERY, AND MEDITATION AS WELL AS VARIOUS POSTURES. THE AUTHORS EXAMINED THE EFFECTS OF THE TIBETAN YOGA (TY) PRACTICES OF TSA LUNG AND TRUL KHOR, WHICH INCORPORATE CONTROLLED BREATHING AND VISUALIZATION, MINDFULNESS TECHNIQUES, AND LOW-IMPACT POSTURES IN PATIENTS WITH LYMPHOMA. METHODS: THIRTY-NINE PATIENTS WITH LYMPHOMA WHO WERE UNDERGOING TREATMENT OR WHO HAD CONCLUDED TREATMENT WITHIN THE PAST 12 MONTHS WERE ASSIGNED TO A TY GROUP OR TO A WAIT-LIST CONTROL GROUP. PATIENTS IN THE TY GROUP PARTICIPATED IN 7 WEEKLY YOGA SESSIONS, AND PATIENTS IN THE WAIT-LIST CONTROL GROUP WERE FREE TO PARTICIPATE IN THE TY PROGRAM AFTER THE 3-MONTH FOLLOW-UP ASSESSMENT. RESULTS: EIGHTY NINE PERCENT OF TY PARTICIPANTS COMPLETED AT LEAST 2-3 THREE YOGA SESSIONS, AND 58% COMPLETED AT LEAST 5 SESSIONS. PATIENTS IN THE TY GROUP REPORTED SIGNIFICANTLY LOWER SLEEP DISTURBANCE SCORES DURING FOLLOW-UP COMPARED WITH PATIENTS IN THE WAIT-LIST CONTROL GROUP (5.8 VS. 8.1; P < 0.004). THIS INCLUDED BETTER SUBJECTIVE SLEEP QUALITY (P < 0.02), FASTER SLEEP LATENCY (P < 0.01), LONGER SLEEP DURATION (P < 0.03), AND LESS USE OF SLEEP MEDICATIONS (P < 0.02). CONCLUSIONS: THE PARTICIPATION RATES SUGGESTED THAT A TY PROGRAM IS FEASIBLE FOR PATIENTS WITH CANCER AND THAT SUCH A PROGRAM SIGNIFICANTLY IMPROVES SLEEP-RELATED OUTCOMES. BACKGROUND: METABOLIC SYNDROME INCREASES THE RISK OF DIABETES AND CARDIOVASCULAR DISEASE. YOGA IMPROVES SOME METABOLIC PARAMETERS, BUT IT HAS NOT BEEN STUDIED IN PERSONS WITH METABOLIC SYNDROME. WE CONDUCTED A RANDOMIZED CONTROLLED PILOT TRIAL TO DETERMINE WHETHER A RESTORATIVE YOGA INTERVENTION WAS FEASIBLE AND ACCEPTABLE IN UNDERACTIVE, OVERWEIGHT ADULTS WITH METABOLIC SYNDROME. METHODS: TWENTY SIX UNDERACTIVE, OVERWEIGHT ADULT MEN AND WOMEN WITH METABOLIC SYNDROME WERE RANDOMIZED TO ATTEND 15 YOGA SESSIONS OF 90 MINUTES EACH OVER 10 WEEKS OR TO A WAIT-LIST CONTROL GROUP. FEASIBILITY WAS MEASURED BY RECRUITMENT RATES, SUBJECT RETENTION, AND ADHERENCE. ACCEPTABILITY WAS ASSESSED BY INTERVIEW AND QUESTIONNAIRES. CHANGES IN METABOLIC OUTCOMES AND QUESTIONNAIRE MEASURES FROM BASELINE TO WEEK 10 WERE CALCULATED. RESULTS: A TOTAL OF 280 PEOPLE WERE SCREENED BY PHONE, AND 93 WITH HIGH LIKELIHOOD OF METABOLIC SYNDROME WERE INVITED TO A SCREENING VISIT. OF THE 68 WHO ATTENDED SCREENING VISITS, 26 (38%) WERE RANDOMIZED, AND 24 (92%) COMPLETED THE TRIAL. ATTENDANCE AT YOGA CLASSES AND ADHERENCE TO HOME PRACTICE EXCEEDED OUR GOALS. IN THE YOGA GROUP, ALL PARTICIPANTS GAVE THE STUDY THE HIGHEST POSSIBLE SATISFACTION RATING, AND THE MAJORITY (87%) FELT THAT THE YOGA POSES WERE EASY TO PERFORM. THERE WAS TREND TO REDUCED BLOOD PRESSURE (P = 0.07), A SIGNIFICANT INCREASE IN ENERGY LEVEL (P < 0.009), AND TRENDS TO IMPROVEMENT IN WELL-BEING (P < 0.12) AND STRESS (P < 0.22) IN THE YOGA VERSUS CONTROL GROUP. CONCLUSIONS: RESTORATIVE YOGA WAS A FEASIBLE AND ACCEPTABLE INTERVENTION IN OVERWEIGHT ADULTS WITH METABOLIC SYNDROME. THE EFFICACY OF YOGA FOR IMPROVING METABOLIC PARAMETERS IN THIS POPULATION SHOULD BE EXPLORED IN A LARGER RANDOMIZED CONTROLLED TRIAL. 2008
