1 5084 182 PIGMENTED EPENDYMOMA, A TUMOR WITH PREDILECTION FOR THE MIDDLE-AGED ADULT: CASE REPORT WITH METHYLATION CLASSIFICATION AND REVIEW OF 16 LITERATURE CASES. EPENDYMOMAS HAVE RARELY BEEN DESCRIBED TO CONTAIN PIGMENT OTHER THAN MELANIN, NEUROMELANIN, LIPOFUSCIN OR A COMBINATION. IN THIS CASE REPORT, WE PRESENT A PIGMENTED EPENDYMOMA IN THE FOURTH VENTRICLE OF AN ADULT PATIENT AND REVIEW 16 ADDITIONAL CASES OF PIGMENTED EPENDYMOMA FROM THE LITERATURE. A 46-YEAR-OLD FEMALE SHOWED UP WITH HEARING LOSS, HEADACHES, AND NAUSEA. MAGNETIC RESONANCE IMAGING REVEALED A 2.5 CM CONTRAST-ENHANCING CYSTIC MASS IN THE FOURTH VENTRICLE, WHICH WAS RESECTED. INTRAOPERATIVELY, THE TUMOR APPEARED GREY-BROWN, CYSTIC, AND WAS ADHERENT TO THE BRAINSTEM. ROUTINE HISTOLOGY REVEALED A TUMOR WITH TRUE ROSETTES, PERIVASCULAR PSEUDOROSETTES AND EPENDYMAL CANALS CONSISTENT WITH EPENDYMOMA, BUT ALSO SHOWED CHRONIC INFLAMMATION AND ABUNDANT DISTENDED PIGMENTED TUMOR CELLS THAT MIMICKED MACROPHAGES IN FROZEN AND PERMANENT SECTIONS. THE PIGMENTED CELLS WERE POSITIVE FOR GFAP AND NEGATIVE FOR CD163 CONSONANT WITH GLIAL TUMOR CELLS. THE PIGMENT WAS NEGATIVE FOR FONTANA-MASSON, POSITIVE FOR PERIODIC-ACID SCHIFF AND AUTOFLUORESCENT, WHICH COINCIDE WITH CHARACTERISTICS OF LIPOFUSCIN. PROLIFERATION INDICES WERE LOW AND H3K27ME3 SHOWED PARTIAL LOSS. H3K27ME 3 IS AN EPIGENETIC MODIFICATION TO THE DNA PACKAGING PROTEIN HISTONE H3 THAT INDICATES THE TRI-METHYLATION OF LYSINE 27 ON HISTONE H3 PROTEIN. THIS METHYLATION CLASSIFICATION WAS COMPATIBLE WITH A POSTERIOR FOSSA GROUP B EPENDYMOMA (EPN_PFB). THE PATIENT WAS CLINICALLY WELL WITHOUT RECURRENCE AT THREE-MONTH POST-OPERATIVE FOLLOW-UP APPOINTMENT. OUR ANALYSIS OF ALL 17 CASES, INCLUDING THE ONE PRESENTED, SHOWS THAT PIGMENTED EPENDYMOMAS ARE MOST COMMON IN THE MIDDLE-AGED WITH A MEDIAN AGE OF 42 YEARS AND MOST HAVE A FAVORABLE OUTCOME. HOWEVER, ONE PATIENT THAT ALSO DEVELOPED SECONDARY LEPTOMENINGEAL MELANIN ACCUMULATIONS DIED. MOST (58.8%) ARISE IN THE 4TH VENTRICLE, WHILE SPINAL CORD (17.6%) AND SUPRATENTORIAL LOCATIONS (17.6%) WERE LESS COMMON. THE AGE OF PRESENTATION AND GENERALLY GOOD PROGNOSIS RAISE THE QUESTION OF WHETHER MOST OTHER POSTERIOR FOSSA PIGMENTED EPENDYMOMAS MAY ALSO FALL INTO THE EPN_PFB GROUP, BUT ADDITIONAL STUDY IS REQUIRED TO ADDRESS THAT QUESTION. 2022 2 6242 37 THE MATRICELLULAR PROTEIN SPARC DECREASES IN THE LACRIMAL GLAND AT ADULTHOOD AND DURING INFLAMMATION. PURPOSE: SECRETED PROTEIN ACIDIC AND RICH IN CYSTEINE (SPARC) IS A MATRICELLULAR GLYCOPROTEIN ABUNDANTLY EXPRESSED IN BASEMENT MEMBRANES AND CAPSULES SURROUNDING A VARIETY OF ORGANS AND TISSUES. IT MEDIATES EXTRACELLULAR MATRIX ORGANIZATION AND HAS BEEN IMPLICATED IN CELL CONTRACTION. HERE, WE EVALUATED THE EXPRESSION OF SPARC IN THE MURINE LACRIMAL GLAND AT ADULTHOOD AND DURING INFLAMMATION. METHODS: LACRIMAL GLANDS OF YOUNG MICE (4-6 WEEKS OLD) AND ADULT MICE (32-40 WEEKS OLD) WERE USED FOR EXTRACTION OF DNA, RNA, AND PROTEIN. THE PRESENCE OF SPARC WAS ASSESSED BY QUANTITATIVE PCR, ELISA, AND IMMUNOFLUORESCENCE MICROSCOPY. 5-METHYLCYTOSINE AND DNA METHYLATION WERE EVALUATED USING ELISA AND BISULFITE GENOMIC SEQUENCING, RESPECTIVELY. THE EFFECTS OF CYTOKINES AND INFLAMMATION IN SPARC EXPRESSION WERE EVALUATED IN VITRO AND IN THE NON-OBESE DIABETIC (NOD) MOUSE MODEL OF SJOGREN'S SYNDROME. RESULTS: THE MRNA AND PROTEIN LEVELS OF SPARC WERE DOWNREGULATED IN LACRIMAL GLANDS OF MATURE ADULT MICE PRESENTING AGE-RELATED HISTOLOGICAL ALTERATIONS SUCH AS INCREASED DEPOSITION OF LIPOFUSCIN AND LIPIDS. OBJECTIVES: THE STUDY OF THE PROINFLAMMATORY ROLE OF URIC ACID HAS FOCUSED ON THE EFFECTS OF ITS CRYSTALS OF MONOSODIUM URATE (MSU). HOWEVER, LITTLE IS KNOWN WHETHER URIC ACID ITSELF CAN DIRECTLY HAVE PROINFLAMMATORY EFFECTS. IN THIS STUDY, WE INVESTIGATE THE PRIMING EFFECTS OF URIC ACID EXPOSURE ON THE CYTOKINE PRODUCTION OF PRIMARY HUMAN CELLS UPON STIMULATION WITH GOUT-RELATED STIMULI. METHODS: PERIPHERAL BLOOD MONONUCLEAR CELLS (PBMCS) WERE HARVESTED FROM PATIENTS WITH GOUT AND HEALTHY VOLUNTEERS. EPIGENETIC REPROGRAMMING AT THE LEVEL OF HISTONE METHYLATION BY URIC ACID WAS INVOLVED IN THIS EFFECT. CONCLUSIONS: IN THIS STUDY WE DEMONSTRATE A MECHANISM THROUGH WHICH HIGH CONCENTRATIONS OF URIC ACID (UP TO 50 MG/DL) INFLUENCE INFLAMMATORY RESPONSES BY FACILITATING IL-1BETA PRODUCTION IN PBMCS. AN APPROPRIATE STATISTICAL ANALYSIS WAS APPLIED. RESULTS: METHYLATION INDICES FOR ALL THE FOUR GENES IN PDAC CASES WERE FOUND TO BE SIGNIFICANTLY HIGHER AS COMPARED TO THAT IN HEALTHY INDIVIDUALS. SPARC MI VALUES WERE FOUND TO DIFFERENTIATE EARLY-STAGE PDAC PATIENTS FROM CP PATIENTS. PDAC PATIENTS WITH THE METASTASIZED DISEASE AND STAGE IV DISEASE WERE FOUND TO HAVE HIGH MI FOR THE SPARC GENE AS WELL AS FOR THE NPTX2 GENE, WHILE A HIGHER UCHL1 METHYLATION INDEX WAS FOUND TO CORRELATE WITH AN ADVANCED STAGE OF THE DISEASE. HIGHER MI VALUES FOR SPARC AND NPTX2 GENES WERE FOUND TO ASSOCIATE WITH POOR SURVIVAL IN PATIENTS WITH PDAC. HOWEVER, CONTINUOUS DEVELOPMENTAL TCE EXPOSURE ALTERED THE METHYLATION OF 274 CPG SITES IN PROMOTERS AND CPG ISLANDS. PATHWAY ANALYSIS OF THE DMRS INDICATED THAT TCE PRIMARILY ALTERED THE METHYLATION OF GENES ASSOCIATED WITH REGULATION OF CELLULAR METABOLISM AND CELL SIGNALING. THE RESULTS DEMONSTRATED THAT CONTINUOUS DEVELOPMENTAL EXPOSURE TO TCE DIFFERENTIALLY METHYLATED BINDING SITES OF PCG PROTEINS IN EFFECTOR/MEMORY CD4(+) CELLS. THERE WERE MINIMAL YET POTENTIALLY BIOLOGICALLY SIGNIFICANT EFFECTS THAT OCCURRED WHEN EXPOSURE WAS DISCONTINUED. WE INVESTIGATED THE CLINICAL AND PHARMACODYNAMIC RESULTS OF DIFFERENT DOSE SCHEDULES OF DECITABINE. RANDOMIZATION FOLLOWED A BAYESIAN ADAPTIVE DESIGN. NINETY-FIVE PATIENTS WERE TREATED (77 WITH MDS, AND 18 WITH CMML). OVERALL, 32 PATIENTS (34%) ACHIEVED A COMPLETE RESPONSE (CR), AND 69 (73%) HAD AN OBJECTIVE RESPONSE BY THE NEW MODIFIED INTERNATIONAL WORKING GROUP CRITERIA. THE 5-DAY INTRAVENOUS SCHEDULE, WHICH HAD THE HIGHEST DOSE-INTENSITY, WAS SELECTED AS OPTIMAL; THE CR RATE IN THAT ARM WAS 39%, COMPARED WITH 21% IN THE 5-DAY SUBCUTANEOUS ARM AND 24% IN THE 10-DAY INTRAVENOUS ARM (P < .05). THE HIGH DOSE-INTENSITY ARM WAS ALSO SUPERIOR AT INDUCING HYPOMETHYLATION AT DAY 5 AND AT ACTIVATING P15 EXPRESSION AT DAYS 12 OR 28 AFTER THERAPY. THUS, WE AIMED TO ANALYZE THE DNA METHYLATION STATUS IN IL1RN, RELA (P65) AND NFKB1 (P50) GENES IN PERIPHERAL BLOOD MONONUCLEAR CELLS (PBMCS) FROM TREATED T2DM PATIENTS (N = 35) AND AGE-/SEX- MATCHED HEALTHY CONTROLS (N = 31). PRODUCTION OF IL-1BETA AND IL-1RA WAS ANALYZED IN PLASMA AND SUPERNATANTS FROM LPS-INDUCED PBMCS. IMMUNOMODULATORY EFFECTS OF IL-1BETA AND IL-1RA WERE STUDIED ON THP-1 CELLS. EPIGENETIC SILENCING OF THE P16 AND P15 GENES BY PROMOTER METHYLATION ARE COMMONLY OBSERVED IN HUMAN EPITHELIAL MALIGNANCIES, INCLUDING HEAD AND NECK SQUAMOUS CELL CARCINOMAS (HNSCC). IN THIS STUDY, A METHYLATION-SPECIFIC POLYMERASE CHAIN REACTION (MSP) WAS USED TO EVALUATE THE METHYLATION STATUS OF THE P16 AND P15 GENES IN 73 HNSCC SURGICAL SPECIMENS. P16 AND P15 GENE METHYLATION WAS ALSO EXAMINED IN 29 PAIRED METASTATIC LYMPH NODES AND 29 PAIRED HISTOLOGICALLY, NORMAL RESECTION MARGIN MUCOSAE. THE QUANTITY OF CELL-FREE METHYLATED P16 AND P15 DNA IN THE PLASMA SAMPLES OF 20 HNSCC PATIENTS AND 24 HEALTHY CONTROLS WAS ALSO EXAMINED USING A FLUORESCENCE-BASED REAL-TIME PCR ASSAY. THE FREQUENCIES OF P16 AND P15 METHYLATION IN THE PRIMARY TUMOUR WERE 49% AND 60%, RESPECTIVELY. IN THIS STUDY, THE DNA METHYLATION ALTERATION WITHIN THE TNFA PROMOTER IN HUMAN GINGIVAL BIOPSIES FROM DIFFERENT STAGES OF PERIODONTAL DISEASE IS INVESTIGATED AND THE REGULATORY MECHANISM OF TNFA TRANSCRIPTION BY DNA METHYLATION IS EXPLORED.
