VARIOUS CLINICAL PRESENTATIONS HAVE BEEN ATTRIBUTED TO THE DISEASE AS A RESULT OF THE DIFFERENT UNDERLYING GENETIC AND EPIGENETIC ALTERATIONS. THE CURRENT STUDY HAS BEEN INITIATED TO STUDY THE ROLE OF AN EPIGENETIC ALTERATION AFFECTING THE PROMOTER OF THE TP53GENE ON CLL PATHOGENESIS AND PROGRESSION. DIFFERENTIAL METHYLATION ANALYSIS BETWEEN PATIENTS AND CONTROLS ALLOWED US TO IDENTIFY ABNORMALITIES IN DNA METHYLATION, INCLUDING HYPERMETHYLATION OF SPECIFIC GENES AND LARGE GENOME REGIONS WITH ABERRANT DNA METHYLATION. UNSUPERVISED HIERARCHICAL CLUSTER ANALYSIS IDENTIFIED TWO MAIN CLUSTERS THAT ASSOCIATED WITH THE CLINICAL, BIOLOGICAL, AND GENETIC FEATURES OF PATIENTS. GROUP 1 WAS ENRICHED IN PATIENTS WITH ADVERSE CLINICAL AND BIOLOGICAL CHARACTERISTICS AND POORER OVERALL AND PROGRESSION-FREE SURVIVAL. BACKGROUND & AIMS: THE ABILITY OF EXOCRINE PANCREATIC CELLS TO CHANGE THE CELLULAR PHENOTYPE IS REQUIRED FOR TISSUE REGENERATION UPON INJURY, BUT ALSO CONTRIBUTES TO THEIR MALIGNANT TRANSFORMATION AND TUMOR PROGRESSION. WE INVESTIGATED CONTEXT-DEPENDENT SIGNALING AND TRANSCRIPTION MECHANISMS THAT DETERMINE PANCREATIC CELL FATE DECISIONS TOWARD REGENERATION AND MALIGNANCY. IN PARTICULAR, WE STUDIED THE FUNCTION AND REGULATION OF THE INFLAMMATORY TRANSCRIPTION FACTOR NUCLEAR FACTOR OF ACTIVATED T CELLS 1 (NFATC1) IN PANCREATIC CELL PLASTICITY AND TISSUE ADAPTATION. METHODS: WE ANALYZED CELL PLASTICITY DURING PANCREATIC REGENERATION AND TRANSFORMATION IN MICE WITH PANCREAS-SPECIFIC EXPRESSION OF A CONSTITUTIVELY ACTIVE FORM OF NFATC1, OR DEPLETION OF ENHANCER OF ZESTE 2 HOMOLOGUE 2 (EZH2), IN THE CONTEXT OF WILD-TYPE OR CONSTITUTIVELY ACTIVATE KRAS, RESPECTIVELY. ACUTE AND CHRONIC PANCREATITIS WERE INDUCED BY INTRAPERITONEAL INJECTION OF CAERULEIN. EZH2-DEPENDENT REGULATION OF NFATC1 EXPRESSION WAS STUDIED IN MOUSE IN HUMAN PANCREATIC TISSUE AND CELLS BY IMMUNOHISTOCHEMISTRY, IMMUNOBLOTTING, AND QUANTITATIVE REVERSE TRANSCRIPTION POLYMERASE CHAIN REACTION. WE USED GENETIC AND PHARMACOLOGIC APPROACHES OF EZH2 AND NFATC1 INHIBITION TO STUDY THE CONSEQUENCES OF PATHWAY DISRUPTION ON PANCREATIC MORPHOLOGY AND FUNCTION. EPIGENETIC MODIFICATIONS ON THE NFATC1 GENE WERE INVESTIGATED BY CHROMATIN IMMUNOPRECIPITATION ASSAYS. RESULTS: NFATC1 WAS RAPIDLY AND TRANSIENTLY INDUCED IN EARLY ADAPTATION TO ACINAR CELL INJURY IN HUMAN SAMPLES AND IN MICE, WHERE IT PROMOTED ACINAR CELL TRANSDIFFERENTIATION AND BLOCKED PROLIFERATION OF METAPLASTIC PANCREATIC CELLS. HOWEVER, IN LATE STAGES OF REGENERATION, NFATC1 WAS EPIGENETICALLY SILENCED BY EZH2-DEPENDENT HISTONE METHYLATION, TO ENABLE ACINAR CELL REDIFFERENTIATION AND PREVENT ORGAN ATROPHY AND EXOCRINE INSUFFICIENCY. IN CONTRAST, ONCOGENIC ACTIVATION OF KRAS SIGNALING IN PANCREATIC DUCTAL ADENOCARCINOMA CELLS REVERSED THE EZH2-DEPENDENT EFFECTS ON THE NFATC1 GENE AND WAS REQUIRED FOR EZH2-MEDIATED TRANSCRIPTIONAL ACTIVATION OF NFATC1. CONCLUSIONS: IN STUDIES OF HUMAN AND MOUSE PANCREATIC CELLS AND TISSUE, WE IDENTIFIED CONTEXT-SPECIFIC EPIGENETIC REGULATION OF NFATC1 ACTIVITY AS AN IMPORTANT MECHANISM OF PANCREATIC CELL PLASTICITY. INHIBITORS OF EZH2 MIGHT THEREFORE INTERFERE WITH ONCOGENIC ACTIVITY OF NFATC1 AND BE USED IN TREATMENT OF PANCREATIC DUCTAL ADENOCARCINOMA. 2017 19 5276 24 PROMOTER METHYLATION OF TUMOR-RELATED GENES IN GASTRIC CARCINOGENESIS. ABERRANT PROMOTER METHYLATION AND SUBSEQUENT SILENCING OF CANCER-RELATED GENES HAS BEEN RECOGNIZED AS AN IMPORTANT PATHWAY INVOLVED IN GASTRIC CARCINOGENESIS. IN FACT, SEVERAL FACTORS ARE BELIEVED TO CONTRIBUTE TO ITS INDUCTION IN GASTRIC EPITHELIA, INCLUDING AGING, DIET, CHRONIC INFLAMMATION AND INFECTION OF HELICOBACTER PYLORI (H. PYLORI) AND EPSTEIN-BARR VIRUS (EBV). HOWEVER, THE UNDERLING MECHANISMS ARE NOT COMPLETELY IDENTIFIED, DESPITE THE BELIEF THAT INCREASED EXPRESSION OR ACTIVITY OF DNA METHYLTRANSFERASES (DNMTS), OR DECREASED DEMETHYLATION ACTIVITY MAY CONTRIBUTE TO THE EXCESSIVE METHYLATION. A GREAT NUMBER OF GENES WITH PROMOTER METHYLATION HAVE BEEN OBSERVED IN GASTRIC CANCER (GC), AMONG WHICH P16INK4A (P16), MUT L HOMOLOGUE 1 (MLH1), EPITHELIAL-CADHERIN (E-CADHERIN), RUNT-RELATED TRANSCRIPTION FACTOR 3 (RUNX3), ADENOMATOUS POLYPOSIS COLI (APC), O(6)-METHYLGUANINE-DNA METHYLTRANSFERASE (MGMT), RAS ASSOCIATION DOMAIN FAMILY 1A (RASSF1A) AND DEATH-ASSOCIATED PROTEIN KINASE (DAPK) HAVE BEEN EXTENSIVELY STUDIED. UNLIKE THE DISTINCT METHYLATION CHARACTERIZATION IN SINGLE GENES, METHYLATION ANALYSIS OF MULTIPLE GENES MAY PROVIDE MORE INFORMATION IN RISK PREDICTION, EARLY DETECTION, PROGNOSIS ASSESSMENT AND CHEMOTHERAPY CHOICE FOR GC. SPECIFICALLY, PARTICULAR MONITORING AND SCREENING SHOULD BE PERFORMED ON THOSE OVER 45 YEARS OLD, WITH PRECANCEROUS GASTRIC DISEASE OR INFECTION OF H. PYLORI OR EBV. AS AN ALTERNATIVE TO TUMOR TISSUES, METHYLATION DETECTION IN PATIENT SERA OR GASTRIC WASHES MAY ALSO BE USED IN RISK PREDICTION AND EARLY DETECTION. HOWEVER, WHAT STILL POSES A GREAT CHALLENGE AS WELL AS A PUZZLE IS THE DETERMINATION OF THE VERY GENES THAT SHOULD BE USED IN METHYLATION ANALYSIS. BECAUSE EPIGENETIC ALTERATIONS ARE NORMALLY REVERSIBLE, DRUGS OR CHEMICAL COMPOUNDS WITH DEMETHYLATING ACTIVITY, SUCH AS 5-AZA-2'-DEOXYCYTIDINE (5-AZA-DC) COULD BE USED IN THE TREATMENT OF PATIENTS WITH MULTIPLE GENE METHYLATION. IN VIEW OF THE ADVERSE EFFECTS OF 5-AZA-DC, DNMT-TARGETED STRATEGY HAS BEEN PROPOSED AND MAY PROVE TO BE MORE EFFECTIVE THAN DEMETHYLATING AGENTS. 2012 20 2008 21 EPIGENETIC BASIS FOR MONOCYTE DYSFUNCTION IN PATIENTS WITH SEVERE ALCOHOLIC HEPATITIS. BACKGROUND & AIMS: SEVERE FORMS OF ALCOHOL-RELATED LIVER DISEASE ARE ASSOCIATED WITH INCREASED SUSCEPTIBILITY TO INFECTIONS WHICH ARE ASSOCIATED WITH POOR PROGNOSIS. THE CELLULAR AND MOLECULAR MECHANISMS RESPONSIBLE FOR THIS ALTERED HOST DEFENSE ARE INCOMPLETELY UNDERSTOOD. METHODS: WE PERFORMED WHOLE BLOOD PHENOTYPIC ANALYSIS AND EX VIVO STIMULATION WITH VARIOUS PATHOGEN-ASSOCIATED MOLECULAR PATTERNS (PAMPS). WE INCLUDED 34 PATIENTS WITH ALCOHOL-RELATED CIRRHOSIS (18 OF WHOM HAD BIOPSY-PROVEN SEVERE ALCOHOLIC HEPATITIS [SAH]), 12 HEALTHY CONTROLS AND 11 PATIENTS WITH CHRONIC ALCOHOL CONSUMPTION WITHOUT SIGNIFICANT LIVER DISEASE. WE ALSO EVALUATED THE TRANSCRIPTOMIC (RNA-SEQ) AND CHROMATIN ACCESSIBILITY (ATAC-SEQ) PROFILES OF CD14(+) MONOCYTES FROM A SUBSET OF PATIENTS. RESULTS: CIRCULATING MONOCYTES AND CONVENTIONAL DENDRITIC CELLS (DCS) FROM PATIENTS WITH SAH DISPLAYED COMPLEX ALTERATIONS CHARACTERIZED BY INCREASED EXPRESSION OF BOTH ACTIVATING AND INHIBITORY SURFACE MARKERS AND AN IMPAIRED PRO-INFLAMMATORY RESPONSE UPON STIMULATION WITH PAMPS REPRESENTATIVE OF GRAM-NEGATIVE BACTERIA (LIPOPOLYSACCHARIDE, PAM3CSK4) OR FUNGAL PATHOGENS (ZYMOSAN). THEIR DECREASED ABILITY TO PRODUCE MORE THAN 1 CYTOKINE (POLYFUNCTIONALITY) UPON PAMP STIMULATION CORRELATED WITH THE RISK OF DEVELOPING INFECTION AT 28 DAYS OR MORTALITY AT 90 DAYS. THE PRESENCE OF ACUTE-ON-CHRONIC LIVER FAILURE IN PATIENTS WITH SAH DID NOT SIGNIFICANTLY MODIFY THE IMMUNE PROFILE OF MONOCYTES AND DCS. MOREOVER, CD14(+) MONOCYTES OF PATIENTS WITH SAH DISPLAYED ALTERED TRANSCRIPTIONAL AND EPIGENOMIC PROFILES CHARACTERIZED BY DOWNREGULATION OF KEY INNATE IMMUNE AND METABOLIC PATHWAYS AND UPREGULATION OF IMPORTANT IMMUNOMODULATORY FACTORS. CONCLUSIONS: IN PATIENTS WITH SAH, THE ALTERED TRANSCRIPTIONAL PROGRAM AND FUNCTIONAL PROPERTIES OF MONOCYTES THAT CONTRIBUTE TO PATIENTS' SUSCEPTIBILITY TO INFECTION HAVE STRONG EPIGENETIC DETERMINANTS. LAY SUMMARY: PATIENTS WITH SEVERE ALCOHOLIC HEPATITIS ARE AT INCREASED RISK OF INFECTIONS, WHICH CONTRIBUTE TO THE POOR PROGNOSIS ASSOCIATED WITH THE DISEASE. HEREIN, WE SHOW THAT EPIGENETIC DETERMINANTS UNDERLY THE IMMUNE CELL DYSFUNCTION AND INAPPROPRIATE RESPONSES TO PATHOGENS THAT ARE ASSOCIATED WITH SEVERE ALCOHOLIC HEPATITIS. 2020
